We at Fairview Health Services are seeking for a driven Licensed Practical Nurse (LPN) to join the hard working Women's Health team at our Edina Clinic. We have approved this position for 68 hours every two weeks and it is benefit eligible! Responsibilities Job Description
The LPN performs delegated nursing and medical functions of data collection and delivery of care to an assigned group of patients, or individualized patient. The LPN works under the direction of a Registered Nurse, or physician/authorized practitioner and in collaboration with other members of the healthcare team. The LPN is responsible for adhering to all policies, procedures, and nursing practice guidelines.
Collects data for patient assessments.
Involves the patient, their support network, and other health team members in capturing relevant data.
Reports unusual data or changes in patient's condition to the RN, or physician/authorized practitioner in a timely manner.
Delivers care as delegated by the RN or physician/authorized practitioner to assigned patient(s) population, or individual patient.
Follows the patient’s plan of care.
Prioritizes work based on patient's acuity, available resources, and patient/support network preferences, in collaboration with other care team members
Performs high and low risk technical procedures, including medications, defined by the clinical setting.
Recognizes when different approaches may be needed based on age or cultural differences.
Implements assigned aspects of the patient's teaching plan.
Optimally communicates compliance and concerns to delegating RN or physician/authorized practitioner.
Takes action to assure continuity of care for all assigned patients.
Responds appropriately to emergent situations.
Communicates optimally with other members of the health care team.
- Communicates verbal and written information in a clear, concise, accurate, and timely manner.
Qualifications
Required
Vocational/Technical Training or Associates Degree
Current MN LPN License
Preferred
One to three years
Previous ambulatory Care experience.
Current BLS Certification
