ALWAYS CARING HEALTH CARE Home Health RN case manager in EL PASO, Texas
As a case manager, you will be required to maintain patient care records, perform QA/QI, maintain and forward all pertinent information to the field staff to provide the most accurate care possible. Must be familiar with Oasis reports and SN as it pertains to home health. Assess patient health problems and needs, develop and implement nursing care plans, and maintain medical records. Administer nursing care to ill, injured, convalescent, or disabled patients. May advise patients on health maintenance and disease prevention or provide case management. Licensing or registration required.
