Position Summary

This position is responsible for providing individualized person centered and recovery focused community-based case management services to adults, children or elders diagnosed with a mental illness and/ or substance use disorder as defined in the Wisconsin Statutes and within standards, ordinances, and policies set forth by the State of Wisconsin and Walworth County. Individuals enrolled in this program are active participants in the Community Recovery Services (CRS) program, Comprehensive Community Support (CCS), and Community Support Program (CSP). In order to provide clinically necessary services, CSP staff work within a program that must operate seven days per week including holidays.

Essential Duties and Responsibilities

This job description reflects management's assignment of essential functions; it does not prescribe or restrict the tasks that may be assigned.

Provide comprehensive community-based case management, psychosocial rehabilitation, skill development, supportive psychotherapy, symptom education, vocational support and assistance with benefits to empower consumers to live in the least restrictive setting with maximum opportunity to function in the community and meet their individual needs.

Provide services for an assigned group of consumers, including face-to-face visits with consumers in their home or in the community. At least 50% of service contacts are to be provided in the home or community, in non-office/non-facility based settings.

Conduct comprehensive initial assessments at the time of consumer admission into the program and assure that urgent needs identified are addressed in a timely manner. Perform ongoing assessment updates to accurately reflect the consumer's status, as required by program mandates.

Provide ongoing clinical assessment of mental status and symptomatology. Adjust treatment interventions to address any immediate health and/or safety concerns, as well as meet the current needs of the consumer in order to foster their recovery.

Facilitate the initial and ongoing development of treatment/recovery plans and assist with the identification of consumer needs, strengths, goals, and their priority in the coordinated plan. Implement and carry out the plan, coordinate and monitor services/treatment, evaluate and assess consumer/family psychosocial difficulties, review and adjust treatment interventions and services as required by program mandates, or sooner as determined by changes to a consumer's status or needs.

Ensure compliance with all program mandates, department requirements, division policies and applicable state statutes and regulations including, but not limited to DHS 34, 36, 51, 55, 92 and 94. Maintain accurate and timely clinical records and documentation in accordance with federal, state and agency requirements.

Continually monitor and assess the current level of service needs and adjust accordingly to ensure the appropriate degree of services is provided. Recommend additional services to support consumers when there is an assessed clinical need, a reduction in service provision, program discharge and referral to an alternative case management program, community agency and alternative supportive services when indicated.

Collaborate with contracted providers and closely monitor purchased services that are provided to consumers. Based on assessment findings make a determination regarding ongoing clinical need and effectiveness of provided services.

When working in CRS, provide initial and ongoing documentation training to contracted service providers. Audit all progress notes received to ensure that the services provided are in alignment with the consumer's treatment/recovery plan and that the documentation is complete in compliance with all Medicaid, state and program requirements. Communicate to division leadership, and when indicated, key contract/fiscal staff members all incidents of provider non-compliance.

Assist consumers in identifying community and other informal individuals to support their recovery.

Administer mandated functional and fiscal screens within state/federal guidelines to determine program eligibility.

Respond to crisis situations, assess level or risk and provide needed services, safety precautions and interventions for their resolution. Develop and maintain an active crisis plan in the consumer's electronic record to ensure that critical information is communicated effectively to crisis intervention after-hours staff, as well as other internal department staff.

Coordinate and monitor services with court related responsibilities and expectations. Prepare court documentation, serve court notices to consumers and provide testimony in court. Complete annual protective placement reviews for persons who are intellectually disabled, elderly or physically disabled.

Participate in trainings in order to update knowledge and skills regarding evidence-based treatments for the treatment and recovery of individuals with mental health and/or substance use disorders to ensure certification/personal licensure. Develop, implement and provide evidence-based services to consumers and their families in order to maintain program standards.

Incorporate current best practice models into daily service provision with individuals, as well as their families and significant others when indicated.

For consumers residing in a residential facility; assess consumer needs, identify areas of habilitation and rehabilitation for providers to address and actively participate in monthly residential division meetings and annual reassessments. Ensure that consumer ability to pay is determined and implemented in a timely manner so that tax levy dollars are used effectively and is the payer of last resort.

Collaborate with other programs/individuals across the department to ensure coordination of care.

Consult, coach and collaborate with team members, families, individuals, providers, internal HHS staff and external providers.

Advocate for consumers and provide community education. Educate consumers, as well as their support system, to enable the consumer to achieve their personal goals and to reach his/her maximum potential.

Participate in Behavioral Health division functions related to the general operation including trainings, supervision, progress meetings for consumers, division/program staff meetings, supervision, resource development, marketing and outreach.

Work closely with the division manager and direct supervisor on personal development, program development, task delegations, new staff orientation and workforce development to ensure that program goals and objectives have been met and staff can appropriately conduct all duties. Monitor personal productivity levels, chart quality and ensure that standards are achieved and that all Medicaid guidelines are met.

Take a leadership role in assigned division administration activities including, but not limited to, timecard approval, program evaluation, electronic chart audits and reviews, division policy and procedure development/updates and preparation for audits conducted by the state, Medicaid and the Office of the Inspector General.

Utilize personal vehicle to transport consumers who need assistance getting to and from appointments and/or other related psychosocial activities.

Monitor and manage email, assign or provide direct assistance for service requests.

Demonstrate a commitment to county safety and risk management efforts.

Qualifications

Education and/or Experience

  • Bachelor's degree in social work, behavioral science, or human services with six (6) months of supervised post degree clinical experience with adults or children diagnosed with a mental illness required.

  • Master's degree in social work, professional counseling, psychology, or behavioral science with 3,000 hours of supervised clinical experience working with children, adults or elders diagnosed with a mental illness is preferred.

  • Experience working with individuals that have a substance use disorder is preferred. Knowledge of Trauma Informed Care principles is essential.

  • In the CSP program, work with Adults and Elders supersedes work with children. For CSP it is also acceptable to have a bachelor's degree in a field other than behavioral sciences with 2000 hours of supervised post degree clinical experience with adults diagnosed with a mental illness.

  • Previous experience with DHS 83, 88 and 89 is preferred when assigned to the CRS program.

  • Proven experience in the ability to read, write and interpret legal documentation as required in Wisconsin Statutes and Standards.

  • Proven experience in ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals as well as ability to compute rate, ratio and percent and to draw and interpret bar graphs.

  • Proven experience with Word and Excel.

Certificates, Licenses, Registrations

Valid Wisconsin driver's license.

Proof of minimum liability insurance coverage.

State of Wisconsin license issued by the Department of Safety and Professional Services (DSPS) in social work or professional counseling is preferred.

Completion of all training/certification requirements listed below within the first three (3) months of employment. Failure to complete program mandated training will be grounds for termination.

  • Comprehensive Community Services (CCS) training requirements as outlined in DHS 36.12(1)(a).

  • Emergency Mental Health Services Program training requirements as outlined in DHS 34.2(8)(b) for all staff.

  • Certification by the State of Wisconsin to administer the Mental Health/AODA Functional Screen and the Children's Long-Term Support Functional Screen for all Non-CSP staff.

  • Community Support Program (CSP) training requirements as outlined in DHS 63.06 (6) for CSP staff only.

WALWORTH COUNTY EMPLOYEE BENEFIT SUMMARY

E - Human Service Professional Employees

BARGAINING UNIT: Human Service Professional Employees Hired on or after January 1, 2012

DATE: January 1, 2019

EMPLOYMENT STATUS: All employees of Walworth County are at-will employees.

BASIS OF PAYMENT: You are paid for actual hours worked plus pay for any accrued benefit time for which you are eligible. You are eligible for overtime or compensatory time in accordance with provisions of the Fair Labor Standards Act.

SCHEDULED TIME OFF: Accrued based on straight-time hours paid each pay period. All accrued scheduled time off hours are available for use, subject to approval of your supervisor. As a full-time employee you earn scheduled time off equivalent to the following:

10 days per year after 1 year of Walworth County Service 15 days per year after 5 years of Walworth County Service 20 days per year after 15 years of Walworth County Service

*certain positions may be considered for additional Scheduled Time Off (STO) based on continuous related experience per Ord. Sec. 15-515 (e)

PERSONAL TIME OFF: Employees will receive 64 hours of personal time off as a lump sum at the beginning of each year (pro-rated for pt and newly-hired employees). At the end of each calendar year, an employee may request to move remaining personal time off hours to the extended sick leave bank. If no request is made, any remaining hours shall be paid out on the employee's last pay date of the calendar year.

EXTENDED SICK LEAVE BANK: Used for time off of work for more serious health conditions that require three or more consecutive days off or when on an approved leave of absence.

HOLIDAYS: Ten paid holidays each year. A p-t employee would receive pro-rated holiday pay based on positions budgeted FTE.

INSURANCE: Employees hired into a .75 FTE or higher position are eligible for Health, Dental, Life, Vision, Short-Term Disability and Long-Term Disability insurance benefits on the first of the month following 30 days of service provided you complete enrollment forms within 30 days of hire. Employees hired into a .50 FTE or higher position are eligible for all benefits listed below except Health insurance. Employees hired into a .49 FTE or less position are eligible to participate in Short-Term Disability only.

Health Insurance: You can choose Tier 1 or Tier 2. Tier 1 is a standard co-pay plan. Tier 2 is a High Deductible Health Plan (HDHP) with a Health Savings Account (HSA). The County will contribute an amount approved by the County Board, payable as a lump sum to your HSA for Tier 2. You may also elect to contribute to your HSA up to the IRS maximum. You may elect either family or single coverage. The monthly premium will be split between the County and the employee.

Dental Insurance: You may elect either family or single coverage. The county shall pay 100% (single or family) of the premium for a full-time and 50% (single or family) of the premium for a part-time employee enrolled in dental insurance.

Life Insurance: The County provides you with term life insurance in an amount equal to $25,000. You have the option of purchasing additional coverage for yourself, spouse and/or dependent children.

Short-Term Disability Insurance: The County provides you with Short-Term Disability Insurance with premiums paid by the employer.

LTD Insurance: The County provides you with Long Term Disability Insurance. The monthly premium is 0.83% of your insured monthly wages. The County shall pay 67% of the premium & the employee shall pay 33% of the premium.

Vision Insurance: You may elect to participate in Vision insurance. The employee electing coverage shall pay 100% (single or family) of the premium for vision insurance.

WISCONSIN RETIREMENT: You are covered under the Wisconsin Retirement System if you are expected to work a minimum of 1200 hours per year for a period of one year or more. The WRS provides you with separation, disability or retirement benefits upon termination. The payment is split between the county and employee, as prescribed by state law. Employees expected to work less than 1200 hours per year will not receive this benefit.

You will receive up to 1 day of pay for the death of your brother-in-law, sister-in-law, son-in-law, daughter-in-law, aunt, uncle, niece or nephew.

JURY DUTY: Your regular wages will be made-whole for any time you are required to serve on jury duty.

MILITARY LEAVE: Your regular wages will be made-whole for up to two weeks for you to meet your annual two-week reserve duty obligation. Extended military leaves are provided in accordance with state and federal laws.

LEAVE OF ABSENCE: You are required to request a leave of absence in writing and a minimum of 15 calendar days prior to your expected absence. All leaves are subject to approval of the Human Resources Department and are at the discretion of the County's representative, except as otherwise required by law. Under emergency situations, the leave papers must be submitted within 5 days after the absence begins.

Periods of absence less than 5 days may be approved by your department head.
