POSITION TITLE: Supports & Services Coordinator or Outpatient Services Clinician (BSW) (Primary responsibility will be with Adults)
POSITION STATUS: 60 hours biweekly / Regular Part-time Delta Professional Unit /Exempt
SALARY RANGE: $18.30/hr---$24.75/hr Supports & Services Coordinator $22.24/hr---$26.44/hr LLBSW pay scale $23.97/hr---$29.67/hr LBSW pay scale Plus 3/4-time benefits Pay rate dependent on Position
RESPONSIBILITIES: This position could provide outpatient psychotherapy, intensive outpatient case management, or Supports Coordination services to a variety of clientele including adults, adolescents, children and families requesting services. This position makes diagnoses, develops treatment plans using the person centered planning process and provides treatment or appropriate referrals. Responsibilities also include monitoring treatment plans to assess recipient's progress and readjusts as appropriate, carrying an active caseload, preparing case histories and needs assessments, providing counseling, follow-up, and aftercare. An employee in this class may be assigned specialization areas or special populations such as adult, child or adolescent. Daytime crisis intervention, groups, family or individual treatment may be assigned.
QUALIFICATIONS: Dependent on position may include: State of Michigan Licensed Bachelors of Social Work (LBSW) or State of Michigan Limited Licensed Bachelors of Social Work (LLBSW). Qualifications for Supports & Services Coordinator will be possession of a Bachelor's Degree in a Human Services Mental Health field (Such as Psychology, Sociology, Criminal Justice, or similar nature) and one year of experience working with persons with mental illness, computer skills applicable. Experience in professional human services is desired.
