Mom's Meals Community Coordinator in Eugene, Oregon

Position SummaryBuild awareness about the Community Supports (CS) program in California and Health Related Social Needs (HRSN) Services in Oregon and Medically Tailored Meals (MTM) from Mom's Meals (MM) throughout communities and the importance of nutrition and its impacts on Medicaid beneficiaries. Possess a passion for ensuring that beneficiaries are aware of and able to access available services that can improve their lives. Create an organized network of contacts and target organizations within internal systems. Engage and participate in the CA Department of Health Care Services (DHCS) and Oregon Health Plan (OHP) county and health plan webinars. Attend and grow network of community events. The successful candidate will drive an increase in MTM referrals from a variety of organizations throughout CA and OR. Build strong relationships with Community Based Organizations, Enhanced Care Management (ECM) Providers, Community Supports (CS) providers, Physicians, Long Term Care (LTC) Facilities, Community Health Workers (CHW), Federally Qualified Health Centers (FQHC), Clinics, Hospital and Health systems, Coordinated Care Organizations (CCO), Outreach and Engagement providers, other HRSN providers, etc.

Ideally, this position will be based in the Northernmost part of California or Portland, Oregon area. It will also be eligible for a company car.

This is one position, but posted in a few locations for more visibility.

Salary range: $60,000-80,312Position Responsibilities may include, but not limited to

  • Outreach to the list of referring groups above to engage and provide an overview of MM services and how to refer members to the program

  • Create and maintain target organization contact list

  • Ensure all contacts and locations for all target organizations are up to date in internal system

  • Offer and schedule onsite (preferred) or virtual in-services with target organizations

  • Be highly visible within the community by engaging in community-based events

  • Attend and participate in county round tables and county and state webinars to represent Mom's Meals

  • Visit Provider groups: Maternal Health practices, behavioral health, community transitions, local health clinics, FQHCs, WIC locations and others and provide overview of MM services

  • Stay up to date on referral workflows and Populations of Focus from CA DHCS, HRSN Covered Populations and contracted health plans

  • Stay up to date with Community Supports integration with closed loop referral systems and Mom's Meals presence within systems

  • Stay up to date on DHCS, OHP and health plan/CCO reporting requirements and work internally to ensure reporting meets requirements

  • Ability to travel 50-70%, including 6-7 overnight stays per month

Required Skills and Experience

  • 2 years of community health work

  • Demonstrated ability to successfully engage community health organizations

  • Proven ability to work with cross functional teams

  • Excellent oral and written communication skills; experience giving presentations

  • Experience using Microsoft Office suite, including PowerPoint and Excel

  • Willingness to travel regionally to events, up to 50% travel

Preferred Skills and Experience

  • Associates or Bachelor's degree

  • Experience working with Medicaid

  • Experience working with food and/or health care organizations

  • Study or work in social services or nutrition

Physical Requirements

  • Repetitive motions that include the wrists, hands and/or fingers

  • Sedentary work that primarily involves sitting, remaining in a stationary position for prolonged periods

  • Visual perception to perform job including peripheral vision, depth perception, and the ability to adjust focus
