PathStone Corporation Housing Case Manage - DS in Fishkill, New York

Summary Purpose for the Position

This position is responsible for providing programmatic oversight and housing case management services ensuring successful transition for people living with HIV from homelessness or at risk of homelessness to permanent housing.

Requirements (Education, Experience, Certification, Knowledge, Skill)

Bachelor’s Degree or equivalent combination of education and experience

Minimum two (2) years experience in human services, preferable with homeless or at risk individuals and families.

Must be able to satisfactorily complete background checks, such as DMV, prior to start.

Must possess a valid NYS driver’s license.

Extensive housing/case management experience and the ability to work effectively with low-income families.

Knowledge of available homeless and at risk services in the community

Knowledge of the Department of Human Services and other non-profits’ systems.

Relevant experience working with persons who are mentally ill, substance abusers, and or homeless.

Interviewing and counseling skills required.

Interacts in a polite, courteous, professional manner. Able to successfully engage with a wide range of clients from diverse backgrounds.

Computer literate

Working knowledge of rental, housing, landlord/tenancy issues preferred.

Bi-lingual skills preferred

Position Responsibilities

Complete intake application and assessment for each family/individual and determine program eligibility.

Assist clients in developing/identifying/evaluating and revising a service plan, milestones and short and long-term goals.

Initiate and monitor client individualized Service Plan, including appropriate Financial Assistance payments.

Provide housing case management services as indicated.

In collaboration with the Housing Resource worker, develop landlord contacts for housing placements; assist in maintaining landlord contact information database.

Establish and maintain a good working relationship with related agencies to ensure Heading Home maintains a comprehensive referral network for clients.

Participate in case conferences when needed.

Accompany clients to appointments as appropriate

Monitor and follow up on program participants’ progress towards meeting goals.

Working Conditions/Environment

Requires flexible work hours including evenings and weekends

Transportation Requirement

Position requires automobile, driver’s license and insurance.
