Parkview North Hospital Medical Social Worker II in Fort Wayne, Indiana
Summary: The Medical Social Worker (MSW) in Palliative Care, Home Health and Hospice assesses patient/family needs and coping skills that support the patient’s unique cultural and spiritual beliefs. Provides holistic patient/family centered care across the treatment settings. Provides education, interventions and community resources specific to the patient/family need. Is a member of the patient's multidisciplinary care team. Participates in discharge planning and treatment planning as needed. Conducts family meetings and advance care plan conversations. Coordinates with the care team and other coworkers to facilitate the patient's goals and achievement of quality and cost-effective outcomes.
Education: Master's Degree in Social Work.
Licensure/Certification: Must have current Indiana LSW or LCSW license. Must have valid drivers license and proof of insurance.
Experience: Minimum of one (1) year's experience in health care with experience in home health care preferred.
Other Qualifications: Demonstrates thorough working knowledge of social work process, community agencies, services, and resources available to assist patients and families in the resolution of emotional , social, and financial difficulties related to change in patient's health status. Requires general knowledge of the normal growth, development, related behaviors, needs, comfort, and range of treatment appropriate to the age of the patient and diagnosis to support the provision of an individualized plan of care. Requires sound analytical and organizational skill to assess patient's needs, plan and coordinate appropriate therapeutic intervention and to manage case load to achieve efficiency and productivity. Must have good verbal and written communication skill.
