Parkview North Hospital Psychiatric Care Navigator in Fort Wayne, Indiana
Psychiatric Care Navigator supports patient/family needs in order to identify psychiatric needs patient/family and provide skill building/psycho-educational interventions as needed. Provides assistance to the multidisciplinary team related to placement, discharge planning, treatment planning, linkage to community resources and crises intervention. Provides discharge planning and appropriate linkage to community resources. Pro-actively promotes Parkview Health service lines, programs and facilities through marketing, community relations and communication efforts. Provides input to the development of service line marketing/communication plans, creates action plans and executes the plans using a variety of methods including print collateral, direct mail, advertising, digital and social media to communicate marketing/communication messages. Plans, organizes and executes special events and serves as a support person for public and media relations. Assists in producing quality, cost-effective outcomes for patients and their families throughout their psychiatric journey. Facilitates the transition of the psychiatric population across the continuum of care. Works collaboratively with patients, families, physicians, office personnel, nursing staff and the interdisciplinary team to overcome barriers to patient progress and desired outcomes. Provides a network of educational, physical, emotional, spiritual, psychosocial and financial assistance to patients and families, including information on community and national resources as needed.
Bachelor's or Master's degree in Social Work, Human Services or other related field. Master's preferred.
Valid drivers license.
2 years in the health care or social service field, psychiatric hospital experience preferred.
