Parkview North Hospital Supervisor Outpatient Therapy in Fort Wayne, Indiana
Position: Supervisor Outpatient Therapy
Qualifications:
Speech-Language Pathologist (SLP):
Must hold a minimum of a Master’s degree in Speech-Language Pathology from a program accredited by the Council on Academic Accreditation in Audiology and Speech-Language Pathology (CAA).
Must hold a Certificate of Clinical Competence (CCC) from the American Speech-Language-Hearing Association (ASHA).
Physical Therapist (PT):
Graduate of an accredited physical therapy program with the following degree requirements:
Minimum of a Bachelor’s degree if graduated prior to 1/1/2016.
Doctorate of Physical Therapy (DPT) required if graduated after 1/1/2016.
International candidates must hold at least a Bachelor’s degree in Physical Therapy (Doctorate preferred).
Licensure and Certifications:
Must hold or be eligible for licensure as a Physical Therapist or Speech-Language Pathologist in the state where services will be provided.
State licensure must be obtained within 90 days of employment.
Must be certified in CPR or become certified within 90 days of employment.
Membership in professional associations (APTA, ASHA) is preferred.
Additional Requirements:
Valid driver’s license required if assigned to drive a company vehicle; proof of insurance required if driving a personal vehicle.
Minimum of three years of experience as a PT or SLP required; previous supervisory experience is preferred.
Must be able to lift 50 pounds and assist with 100 pounds shared lifting.
Strong verbal and written communication skills are required.
