Parkview North Hospital Supervisor Social Work in Fort Wayne, Indiana

Under the direction of department leadership, is responsible for overseeing efficient operation of assigned coworkers including assuring standard work, efficient and high quality outcomes, appropriate length of stay, cost effective care and accurate documentation. Responsible for the oversight of documentation in Epic and assures documentation is accurate and documented in the appropriate location within Epic. Assures that discharge planning is started according to department policy and is efficient, appropriate and well documented. Rounds on staff routinely and looks for opportunities for improvement to support cost effective quality care and transition to the next level of care. Supports department leadership in the daily operations of the department; and in mentoring and professional development of staff. Responsible and accountable for translating the mission, strategic goals and department priorities. Supports and maintains a culture of continuous process improvement as related to care coordination and case management. Works collaboratively with the health care team to remove barriers and achieve the desired outcomes for patient/families. Develops informational and educational content for department meetings as required. Directly supervises and oversees assigned coworkers staff including hiring, training, performance appraisals, staff schedules and appropriate disciplinary action, in conjunction with departmental leadership. This position will require travel to support all Social Workers in Case Management in our Community hospitals

Must hold a Master of Social Work (MSW) or Master of Science in Social Work (MSSW) degree.

Must have current Indiana and Ohio LSW or LCSW License.

Must have three years recent full-time social work experience in an acute care setting, along with acute care case management experience, and process improvement experience. Must have previous manager or supervisor experience.

Must have a strong foundation in leadership principles and application. Makes decisions in an ethical and effective manner. Must have excellent people skills and the ability to problem solve efficiently and effectively. Holds co-workers accountable for outcomes while creating a positive work environment. Excellent written and verbal communication skills. Possesses strong critical thinking skills. Ability to lead and direct co-workers in a positive and professional manner. Demonstrates continued professional development. Strong working knowledge of information technology. Ability to work collaboratively to achieve desired outcomes. Uses creativity, initiative and risk taking in carrying out responsibilities and introducing new ideas or programs; adapts methodologies in accordance with the current strategies and technology.
