    Position Summary

    This is a professional position responsible for serving as the Incident Management lead for the Region. This position independently performs a variety of functions related to incident management including ensuring CMS compliance with provider reporting of critical and reportable incidents within required timeframes, ensuring incidents are entered into the Agency’s Incident Management System within required timeframes. Additionally, this position is responsible for follow-up needs identified through the Data Analysis team and for documenting trends identified when reviewing and closing reportable incidents. This position will also ensure that incident reports are obtained for individuals who seek medical care through emergency room visits and will collaborate with providers and waiver support coordinators in improving the reporting compliance.

    The Work You Will Do

  • Ensure that incidents reports received from providers are identified using the correct incident reporting classification and are complete prior to entering the incidents into the Agency’s Incident Management System (IMS). Follows up with providers to collect missing information, ensuring the follow up reports are submitted by the provider within the required five (5) day timeframe and are entered into IMS and that incidents are closed.

  • Ensures that an associated critical incident is submitted by the provider for any intakes closed with verified findings. Ensures that wellness visits are completed for allegations of Abuse, Neglect or Exploitation or Sexual Misconduct that occur within licensed residential facilities, supported living arrangements or Adult Day Training program. Ensures review of allegations closed with no indicators or not substantiated to identify any client/ provider trends. Enters associated progress notes into IMS and submits intakes for closure by State Office.

  • Conduct follow up activities for providers and clients who are identified through the Data Analysis Team to ensure health and safety needs are being addressed, to assess whether technical assistance or remediation is warranted for providers and recording follow up activities on designated SharePoint site.

  • Based upon review of incident reporting data, work with Incident Management Supervisor and regional trainers in developing training curriculums to focus on reduction of incidents and best practices for providers in reducing/ eliminating incidents.

  • Attend meetings, conference calls, training, workshops/work groups and other miscellaneous activities as needed including participating in the quarterly Qlarant and provider meetings at the request of the Incident Management Supervisor. Works on special projects and other assignments through completion, and meeting deadlines performs other duties as assigned.

    Knowledge of:

  • Incident management system, Incident reporting requirements, Zero Tolerance Operating procedure.

    Ability to:

  • Locate, read and understand, and train others about Agency policy/operating procedures and rules.

    Demonstrate Skills in:

  • IMS, APD iConnect, FSFN, Microsoft Office Suite (TEAMS, Excel, Word, SharePoint) and other Agency implemented software.

    Background Screening Requirement

    It is the policy of the Florida Agency for Persons with Disabilities that applicants for employment undergo Level 2 employment screening in accordance with the requirements of Chapter 435, Florida Statutes, as a condition of employment or being permitted to serve as a volunteer.
