Description and Responsibilities
Open Sky’s Adult Family Care supports individuals over the age of 16, elders and adults with and without disabilities who cannot live alone safely. The individuals served in the program are in need of assistance with one or more activities of daily living and are cared for by either family members or non-family members. The AFC program provide caregivers with several supports such as nursing supports, professional case management, clinical consultation/training, and 24-hour emergency support. With these supports in place, we are able to aid in the caregiver’s ability to care for their loved one.
The LPN Case Manager is a part of the multi-disciplinary team that assures quality care is delivered to each person through assessments and treatment planning. They will develop the member's interim and final plan of care in collaboration with member or responsible party, members of the MDT as well as other individuals designated by the member, which will be reviewed on an ongoing basis. The LPN Case Manager will also monitor the participants health status and complete/submit semi annual participant health status reports to the members PCP, review the PCP Summary Form, Plan of Care, and RN progress note for each on-site visit or encounter and upon significant change.
Other Key Responsibilities:
Complete the medical components of participant’s files, maintain thorough and accurate medical records and documentation and coordinate all applicable clinical assessments and clinical evaluations in the Electronic Health Record.
Coordinate the delivery of AFC with any other health services or supportive services the member is receiving from MassHealth or other agencies or organizations, including but not limited to, visiting nurse services, therapy services, Department of Developmental Services, Department of Mental Health, and Massachusetts Rehabilitation Commission services.
Work collaboratively with the AFC Coordinators to provide case management and ongoing medical teaching and training on health and aging to AFC families and Caregivers.
Conduct regular monthly or every other month (depending on level of care) in-home visits and monitor family and participant’s home’s compliance with Adult Foster Care regulations set forth by Mass Health.
Qualifications
Licensed Practical Nurse in the State of Massachusetts, required.
High School Diploma, GED or equivalent, required.
Valid Drivers License, acceptable driving history and reliable transportation, required.
