SUMMARY DESCRIPTION\ The primary focus of this position is to ensure that participants in the Sand Hill Adult Day Program receive proper care and support.

ESSENTIAL DUTIES & RESPONSIBILITIES\ Responsibilities and duties of the SHADP Nurse shall include, but not be limited to:

  • Direct the provision and quality of the nursing services provided to participants while in attendance at the SHADP including control, security, and administration of medications.
  • Proactively address the medical needs of all SHADP members, prioritizing critical cases and ensuring consistent care (e.g. managing member diabetic care).
  • Coordinate and integrate the medical service with other participant care services to provide a continuum of care for the participant within the scope of services provided by the SHADP.
  • Conduct regular health checks to ensure members well-being. For example: (for new members: check vital signs daily for the first five days; ongoing checks: perform weekly blood pressure monitoring and monthly weight assessments).
  • Obtain information from the participants physician as it relates to attendance and participation in the SHADP and maintain this information in an appropriate client medical record.
  • Monitoring the health status of the SHADP center\'s personnel through direct observation.
  • Maintain accurate and timely nursing summaries, ensuring they reflect member care and health status which may be documented by any other staff members. (e.g., ensuring that notation and documentation in each participants medical chart is dated and signed by the person making such notation or that the corresponding documentation is entered). Frequency: complete and submit monthly summaries in line with SHADP guidelines.
  • Ensure prevention of infection, accident, and injury, and assist each participant to maximize his/her health capabilities.
  • Participate as a SHADP representative on policy development and administration in areas including, but not limited to, pharmaceutical, infection control, and quality assurance.
  • Assist with lunch supervision duties, focusing on preventing health risks, such as choking, and ensuring dietary needs are met.
  • Collaborate with aides in providing personal care activities, ensuring dignity and comfort for members.
  • Engage members in activities and personal interactions, especially those observed not participating. (e.g. encouraging and assisting members with games, puzzles, and social interactions.
  • Welcome members at the door and maintain constant supervision, ensuring safety and prompt response to needs.
  • Engage in ongoing professional development and training to enhance nursing skills and knowledge, especially in areas relevant to SHADP members needs.
  • Assist in the staff training and staff development program. Conduct educational/informational programs for staff and Certified Nursing Assistants, evaluating their skills as they pertain to medical participant care duties and providing consultation to staff.
  • Maintain open and constructive communication with the supervisor, providing updates on member care and any arising challenges. (e.g., notify the SHADP Program Director when, in the nurses professional judgment, there has been an incident and/or a change in a participants condition that warrants notification). Uphold personal and professional confidentiality standards.
  • Ensure that qualified nursing staff is present in the SHADP facility at all times when a participant is present.
  • Participate in participant quality assurance programs that include providing means of evaluating the nursing care of participan s in relation to structure, process, outcomes, and recipient satisfaction.
  • Assisting with the development, maintenance, review, and updating of written, policies and procedures, organizational plans, and a quality assurance program.
  • May be designated to act in the capacity of the SHADP Program Director during periods of absence of the Program Director.
  • Maintain an accurate filing system relative to client information, documenting all contact, and ensuring document control using both written and electronic records that will satisfy regulatory and professional standards.
  • As part of a multidisciplinary team within the SHADP, must acknowledge and protect the rights of individual participants as recipients of SHADP services.
  • Must ensure continuous and ongoing responsibility to provide a safe, secure environment that will enable participants to function at their optimum levels while in the SHADP facility.
  • Contributes to team effort by working collaboratively with all SHADP staff, contributing to a positive and supportive workenvironment, and sharing insights and suggestions to improve member care and staff workflows.
