The Adult Primary Care (APC) clinic is located at 132 Jefferson St., Hartford Hospital. APC Clinic has over 9,000 patient visits annually with an average of 40-50 patients per day. We are a busy outpatient ambulatory care clinic setting at a teaching hospital. We see patients for preventative annual care as well as patients with a wide range of health problems such as diabetes, high cholesterol, asthma/COPD and hypertension. Staffing includes attendings, APRNs, Residents, Psychologist, Social Worker, Nutritionist, Pharm D, RNs, MAs and MAAs. APC facilitates an ongoing relationship between patients and clinicians and fosters participation by patients in decision making about their health and their own care. The majority of our patients are bilingual with Spanish being their primary language and have health coverage under Medicaid. APC provides opportunities for disease prevention and health promotion as well as early detection of problems. APC guides patients through the health system, including appropriate referrals for services to other health professionals. We are recognized as a Patient-Centered Medical Home.* *Job Summary: The Referral Coordinator is responsible for coordinating incoming and outgoing referrals at Adult Primary Care. Manages new patient referrals being sent to the Adult Primary Care Clinic both internally and externally. In collaboration with Central Scheduling colleagues, reviews schedules daily for openings to move patients from the waiting list, reschedule list or incoming referral work queue in order to optimize schedule utilization. Reviews each incoming referral for completeness, ensures necessary documents are received and the referral meets the internal guidelines for an appointment. Monitors outgoing referrals and orders for accuracy and complete information. Ensures patients have been cleared for specialty services, including handling prior authorizations. Point person for all hospitals, departments and other health care facilities pertaining to questions/concerns on all provider orders and referrals. Provides support to clinical staff in order to facilitate the administrative components of clinical referrals for various services. Acts as a liaison between hospital, physicians, health plans, vendors and patients, or other referral services. * * * * * *Job Responsibilities: * * * Registers and schedules patients * Collaborates and communicates with the clinical team to ensure continuity of medical care, to include obtaining, clarifying, validating referral requests, scheduling referral orders and obtaining prior authorizations when indicated. * Responds to all requests (email, website, phone, other) and inquiries to best coordinate a patient’s appointments and needs. * Maintains referral work queue in a timely manner, updates and closes referrals as appropriate. * Contacts patients, families, medical providers and/or facilities to foster smooth transitions of care. * Assists with the creation of standard work for incoming and outgoing referral coordination. * Reviews referring and transfer documents for accuracy and adherence to regulatory compliance and assuring the transitional care processes are implemented. * Consistently communicates with team to make sure all issues and problems are seamlessly handled so that both the patient and the referral sources are satisfied with the result and process. * Responds to internal and external communication timely and accurately. * Participates in meetings and performance improvement activities to ensure cost-effective utilization of resources within own control and to provide quality service to organization through communication, cooperation and collaboration. * Performs other related duties as required. * High School diploma or higher preferred. * Minimum of 1 year as a medical receptionist or previous care coordination experience required. * Direct customer service in the medical field preferred.
