DESCRIPTION OF WORK The Supports Coordinator develops treatment plans and enrolls consumers into Behavioral Health/Intellectual Disabilities/Early Intervention/Autism programs where appropriate. As a caseworker, you will develop life management plans and act as an advocate for the consumer to ensure appropriate treatment as approved. You will also develop Individual Support Plans (ISP) with detailed goals, and locate providers in relation to the ISP. The caseworker will maintain files, records, and other documentation for the continuity of care in a timely and organized manner. In addition, you will routinely document contact with the consumer, family, and service providers.
Duties are not limited to desk work and may include frequent standing, climbing steps (when no step assistance is available), and walking distances. Caseworkers may work more than eight hours per day, frequently driving in and out of the county, and will occasionally travel overnight. Caseworkers manage multiple tasks, paying close attention to details under a constantly changing work environment which includes completion of paperwork and meeting daily strict deadlines.
Work Schedule and Additional Information:
Full-time employment. Work hours are 8:00 am to 4:30 pm, Monday - Friday, with a 1-hour lunch. Occasional overnight and out of County travel. REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY QUALIFICATIONS Minimum Experience and Training Requirements: Two years of experience as a County Social Services Aide 3 and two years of college level course work which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or A bachelor's degree which includes or is supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Services Aide 3 or in a similar position performing paraprofessional case management functions. Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits. Other Requirements: PA residency requirement is currently waived for this title. Hiring preference for this vacancy will be given to candidates who live within Lackawanna, Monroe, Pike, Susquehanna, and Wayne counties. This position falls under the provisions of the Child Protective Services Law.
