Cape Abilities Case Manager CBDS in Hyannis, Massachusetts

GENERAL SUMMARY Writes & manages Individual Service Plans & quarterly reviews in accordance with individual\'s goals & regulation requirements. ESSENTIAL JOB FUNCTIONS Coordinates & creates all Individual Service Plans annual & semi-annual reports in HCIS format based on daily & monthly summaries provided by Community Resource Specialists. Assists with scheduling & acquiring all necessary documentation for Individual Service Plans & team meetings. Manage all documentation that pertain to Individual progress notes & make changes in accordance to Individual Service Plans outcomes &/or goal modifications to assist Community Resource Specialists to track progress. Attends all Individual Service Plans meetings & interfaces with human service agencies to assist individuals. Manages all Individual case books & documentation to assure compliance with agency & state requirements. Interfaces & shares objective development with other relevant departments in the agency prior to Individual Service Plans meetings. Provides assessment & orientation packets to Community Resource Specialists. Oversees & evaluates participant progress through meetings with Individuals & Specialists & report findings to CBDS Coordinator. Maintains monthly schedules of program participation in community activities & services. Attends all related training & information meetings, as required. Serves as m&ated reporter for suspected cases of abuse &/or neglect to the Disabled Persons Protection Commission (DPPC). OTHER DUTIES & RESPONSIBILITIES Performs additional duties, as assigned. PREPARATION, KNOWLEDGE, SKILLS & ABILITIES Bachelor\'s degree. Minimum 3 years prior experience working with adults with disabilities including conducting assessments, performing community outreach & managing job placements. Valid driver?s license in the state of Massachusetts. Knowledge of funding sources & agencies. Attention to detail.
