State of Massachusetts Community Licensed Practical Nurse II in Hyannis, Massachusetts

The Cape & Islands Adult Community Clinical Services (ACCS) operated by the Department of Mental Health (DMH) is seeking a dynamic Licensed Practical Nurse II (LPN II) to work as a direct care service provider in both community and residential settings while maintaining a person centered, trauma informed atmosphere and professional performance. The incumbent to this position will be involved in assisting individuals to meet their rehabilitation goals by actively participating in developing and implementing client treatment planning and working with clients to secure and maintain appropriate community supports, in addition to other duties as assigned. Transporting clients to and from appointments or activities using a program vehicle or their own vehicle is necessary for this role. Please Note: The position is 40 hours per week. The selected candidate will work on day shift (8:00am - 4:30pm). The days off are Sunday and Monday. Employees in this position will be required to work mandatory overtime. _Duties and Responsibilities (These duties and responsibilities are not all inclusive):_ * * Coordinates medication services which include following through with physician orders, prescriptions, filling out Residential Information Sheets and checking to ensure that each coincides, ensuring that medications are sufficient to last until the next appointment, writing self-medication check off sheets, and posting medication changes to ensure clients receive the appropriate medication regimen. * Monitors, reports and records client’s response to current medication and treatment regimens and reports findings to the supervisor for follow up with appropriate medical practitioner. * Educates clients and staff about medications and their effects by utilizing in-service trainings, their own professional knowledge and resources located at the program to ensure that they have a basic knowledge and understanding of such. * Administers medications and treatment orders as prescribed by an appropriate medical practitioner. * Accompanies clients and ensures that clients receive emergency care, as needed. * Maintains and monitors first aid and infection control supplies to ensure that everything is labeled, kept in order, and that adequate supplies are on hand, and have not expired. * Engage persons, individually and/or in group, in the development of meaningful activities, such as recreational, educational, health and wellness activities within the program and the community to foster an improved quality of life. * Provide supportive counseling and maintaining regular contact with persons served and families via outreach in their home or other community environments. * Interpret and communicate all pertinent information, attending team and staff meetings and informing supervisor or designee of any critical information. * Involve persons served and all identified natural supports and collateral providers in the process of treatment planning * Maintain person served records in a timely manner by documenting in EMR and paper record and completing all other related forms to ensure compliance with DMH regulations and Rehab Option requirements. Provide culturally and linguistically competent services that are also sensitive and responsive to religious practice, sexual orientation, gender differences, physical disabilities, as well as services that are age and developmentally appropriate, including young adults and elders. * Foster a professional, courteous and respectful environment in dealing with persons served, families, coworkers and other service providers. *_Required Qualifications:_ * Ability to follow written and oral instructions * Capacity to perform basic mathematics. * Proven ability to gather information through observing and questioning individuals. * Talent for communicating effectively in oral expression, writing clearly and concisely, and developing ideas in logical sequence. * Skilled in interacting with people who are under emotional stress with the ability to maintain a calm manner in stressful or emergency situations. _Preferred Qualifications: _ * Experience working in a Community Mental Health Setting * Experience with DMH regulations and program procedures. * Experience working with individuals with serious, persistent mental illness, substance abuse and/or who have experienced homelessness. * Given the population served, bilingual or multilingual fluency. *Special Requirements:* * Possession of a current and valid class D Motor Vehicle Operator’s license required for this function. * Transporting clients to and from appointments or activities using a program vehicle or their own vehicle is necessary for this role. _DMH Vision and Mission:_ The Department of Mental Health, as the State Mental Health Authority, assures and provides access to services and supports to meet the mental health needs of individuals of all ages, enabling them to live, work and participate in their communities.
