Abbott House Aftercare Coordinator in Irvington, New York

Job SummaryThe Aftercare Coordinator is responsible for discharge planning and associated aftercare services planning of each youth entering an Abbott House QRTP Group Home placement. Goals related to discharge and aftercare must be developed and presented in the first support plan and in every subsequent support plan thereafter while in the QRTP or EMPOWER placement. The Aftercare Coordinator will develop objectives that are measurable and may change throughout the youth’s placement at the Group Home to reflect progress toward achieving the goals. The Aftercare Coordinator will provide required QRTP services for all Group Home youth for a minimum of six months post-discharge or step-down.About Abbott HouseAbbott House is an innovative community-based organization that helps children, families, adults and people with intellectual and developmental disabilities with complex needs build lasting foundations for a promising future. We operate programs in the New York Metropolitan area and Hudson Valley.Key ResponsibilitiesExpectations for in-facility discharge and aftercare planning include the following:Create the youth’s discharge plan in conjunction with the youth’s permanency teamConduct face-to-face meetings with youth twice per month (one must include the family/discharge resource.Refer the youth’s family to community resources based on a needs assessmentFacilitate permanency/support team meetings no less than monthly with key members of the agency staff, the youth, the youth’s family, and other community resource members.Expectations for post-discharge aftercare services include the following:Provide and/or facilitate the services identified in the final support plan (discharge plan)Engage the community upon program discharge from residential care to maintain successful transition to home and community or, in the case of a step-down, engagement with the program receiving the youthIdentify youth development and vocational resources in the home communityEstablish a network of Supervised Setting Programs (SSPs)2, when needed, to move youth with permanency issues to a lower level of care to transition into the communityExpedite youth transition into educational or vocational programs as appropriateAbout YouYou are the ideal candidate if you are enjoy working in the social services field while empowering underserved communities. Being compassionate, reliable, self-motivated, and a team player are critical attributes to be successful in this role.Master’s Degree in a related field preferred. Minimum Salary: 45,000 Maximum Salary: 47,000 Salary Unit: Yearly
