But we didn't stop at providing housing. Breaking Ground's programs and services help people experiencing street homelessness - especially those who have been on the streets the longest - to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home.

Breaking Ground has secured funding to implement a Health Care Pilot in Queens County, New York, which aims to improve connection to primary care and other resources among people experiencing homelessness, while demonstrating that doing so can significantly decrease costs for the healthcare system. The pilot will be rolled out within the Elmhurst/Corona neighborhoods with potential for further expansion after the three-year pilot period.

Reporting to the Program Director, the Program Coordinator, Queens Health Care Pilot oversees a team of Case Managers and Consumer Advocates with lived experience. The Program Coordinator and team provide services to approximately 141 clients this project will serve over the three-year period. The Program Coordinator engages, assesses, and coordinates healthcare and other wrapround services (e.g., legal services, food pantries, transitional housing) for the target population, many of whom are un- and under-documented. The Program Coordinator assists in building excellent relationships with community partners and provides necessary reports as required for this data-driven initiative.

This position will work onsite at a program-site/in the field full-time.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Supervise a hybrid Case Manager/Consumer Advocate team. Provide regular supervision sessions and training

  • Promote a team culture that emphasizes housing first and harm reduction and puts an emphasis on motivational interviewing

  • Identify and enroll clients that meet the target population definition

  • Engage, assess, and intervene with the pilot enrollees to better improve health outcomes by connecting them to primary care, specialty care, substance use/harm reduction care, housing, and other services such as legal assistance

  • Identify and establish excellent relationships with community partners, including Elmhurst Hospital

  • Work with program management to create linkage agreements with 12-20 community partners

  • Work with the pilot team to creatively engage especially hard to reach and vulnerable clients, many of whom have complicated citizenship situations.

  • Serve as the main point of contact Janian psychiatry and primary care providers assigned to the pilot program, including coordinating care and scheduling appointments

    MINIMUM QUALIFICATIONS:

  • LMSW preferred; extensive experience working with the target population may be substituted for a master’s degree

  • Minimum 3 years of experience working with homeless, mentally ill, undocumented, and/or substance using populations

  • Case management experience strongly preferred

  • Previous supervisory experience preferred

  • Preferred training in CPR and First Aid, Opioid Overdose Prevention, crisis intervention, de-escalation

  • Proficiency in motivational interviewing techniques preferred

  • Superb collaborative skills; Ability to work successfully with a wide range of internal and external stakeholders

  • Creativity, strong problem-solving ability

  • Ability to delegate and motivate both staff and clients

  • Ability to communicate (verbally and written) with diverse populations and stakeholders

  • Proficiency with Microsoft Office Suite programs including Word, Excel, Outlook, and Microsoft Teams; proficiency with property management and case management software

  • Valid NY or NJ driver’s license with a good driving record, as defined by Breaking Ground, is a condition of employment

  • Must be willing to work flexible hours

  • Bilingual preferred
