DESCRIPTION OF WORK

As an Aging Care Manager 1, you ill be participating in formal and informal training sessions which provide the methods, procedures, rules, and regulations that govern the program and the internal operations of the area agency on aging to which you will be assigned. You will be responsible for assessing needs of older adults, developing care plans, and arranging services to meet their needs. Work includes completing consumer assessments, preparing forms for eligibility determination, and connecting consumers with appropriate service agencies. Additional responsibilities include, but may not be limited to: Educating consumers and their families about the programs policies and procedures Assessing consumers change of condition and arranging additional or canceling of services Arranging placement of participants in a Domiciliary Care Home Completing routine monitoring visits Maintaining records Relaying updates and changes in Social Security benefits to consumers Assisting with assessments

Work Schedule and Additional Information: Full-time employment Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch. QUALIFICATIONS Minimum Experience and Training Requirements: Three years as an Aging Case Aide 2; or A bachelor's degree which includes or is supplemented by twelve (12) credits in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training.

Applicants will be considered to have met the educational requirements once they are within 3 months of graduating with a qualifying degree. Hiring preference for this vacancy may be given to candidates who live within Cambria County. Legal Requirements: A conditional offer of employment will require drug screening and medical exam.

This position falls under the provisions of the Child Protective Services Law.
