This position has a pay rate of $41,808.00 annually

The Intensive Case Manager (ICM) provides appropriate intensive case management services to refugees or other eligible populations in the US for less than 5 years. ICM fosters the successful and integration of especially vulnerable refugees. The ICM program aims to effectively support vulnerable individuals that have difficulty navigating systems and attaining self-sufficiency in their new or existing community. The ICM case manager will provide intensive support to large families, women-at-risk, elderly refugees, refugees that identify as LGBTQI, refugees with significant physical or mental disabilities, and other vulnerable populations as needed.

Duties and Responsibilities

  • Provide intensive case management to address needs that extend beyond the initial resettlement period, specifically focusing on medical, educational, housing, and clients special needs.

  • Develop individualized, holistic service plans based on client needs.

  • Conduct RHS-15 mental health screening for all ICM clients and makes timely, appropriate referrals to mental health care providers.

  • Network and develop personal contacts with other social service providers, including both mainstream providers and those that serve refugees exclusively. Use contacts and networking to create and maintain a database of client services to supplement those that Samaritas provides.

  • Develop and facilitate periodic community support groups for Samaritas clients to promote community connections and teach healthy coping strategies during the integration experience.

  • Arrange client appointments, provide schedules to clients and other individuals who may be assisting clients (e.g., Samaritas case managers, interpreters, volunteers, etc.) and provide transportation for New American services as required or needed, in compliance with applicable child safety laws.

  • Provide ongoing orientation about health related issues, including Medicaid education, identifying and selecting physicians who provide language appropriate services, and basic public health counseling.

  • In coordination with Volunteer Coordinator, refer clients to one-on-one volunteer mentoring program.

  • Assist ICM clients in resolving emergency issues.

  • Participates in in-service training and conferences related to the programs.

  • Conforms with and abides by all regulations, policies, work procedures and instructions.


Job Qualifications

Education, Training, and Licensure/Certification

  • Bachelor’s degree in social work, psychology, public health or community services preferred, or equivalent experience working with refugees or other vulnerable populations

    Experience

  • Two years of previous direct experience in a culturally diverse environment or community services preferred, or an equivalent combination of education and experience

    Additional Work Requirements

  • Automobile travel required. Valid unrestricted driver’s license with good driving record and reliable personal transportation suitable for safely transporting clients. Valid proof of automobile insurance
