Providence St. Joseph Health Care Management Coordinator - Hematology Oncology in Kennewick, Washington

Description The Care Management Coordinator is responsible for patient care management within a team-based care model. With a focus on specific patient populations, the Care Management Coordinator coordinates patient care, manages referrals, provides in basket support for providers and members of the care team, performs outreach and scheduling, and collects patient reported outcomes including screening. This position provides department administrative support including department data reporting and analytics, administrative assistance, and supports process improvement initiatives. Required Qualifications: Within 60 days of hire: ALL DEPARTMENTS: MA-Registered Within 1 year of hire: POPULATION HEALTH ONLY: Washington State Community Health Worker Certification Within 1 year of hire: ONCOLOGY ONLY: Oncology Patient Navigator Certification 3 years Care coordination, care/case management, program coordination, community health, healthcare administration/setting. Preferred Qualifications: 3 years Application of evidence-based practice/guidelines. 3 years Experience in specialty/area of practice Oncology experience Why Join Providence?
