Job Title: Remote Case Management Assistant
Job Summary:
We are looking for a dedicated Remote Case Management Assistant to help facilitate patient transitions of care and discharge planning for ambulatory patients. Your role will involve coordinating communication between care providers, patients, families, and external vendors to ensure timely and efficient discharges.
Required Skills:
Experience with Utilization Review (UR) transmission.
Ability to manage a high volume of faxes and voicemails.
Proficient in data entry and documentation.
Knowledge of Epic software is required.
Other Details:
Must live in Oregon or Nevada
Must be open to rotating weekends

Pay and Benefits
The pay range for this position is $19.00 - $19.00
