Ministry Support (Diakon Lutheran Social Ministries) Director of Social Services I in Lewisburg, Pennsylvania

*Category/Shift/Hours per 2 week pay period: * Full-Time / Day Shift / 80 hours per pay period

Buffalo Valley Lutheran Village, a Diakon Senior Living Community --- Located in Lewisburg, Pennsylvania, Buffalo Valley is seeking skilled and caring individuals to be part of Diakon's Many Hands One Heart team. Our community features a vibrant community of senior living homes, a personal care center and a skilled nursing and rehabilitative center. General Responsibilities:

  • Responsible for planning, organizing, and directing the social services function.
  • Determines work procedures, prepares work schedules, and expedites workflow.
  • Researches and documents new resident/client's social history and completes an admissions note; schedules conferences with residents' or clients' families as required.
  • Updates progress notes quarterly; concentrates on the psycho-social and psycho-physical needs, adjustment, and discharge plans of residents and clients.

     

Education/Training: A Bachelor's degree in Social Work or Human Services; including but not limited to sociology, gerontology, special education, rehabilitation counseling and psychology required; a Master's degree in Social Work or a related field preferred.

 

Experience: One (1) year of supervised social work experience in a health care setting working directly with individuals required.  Supervisory experience normally required.

 

ADA: Ability to sit, stand, lift, bend, reach, push, and pull items weighing up to 25 lbs.; visual and auditory skills.
