Our Lewiston office has a full-time position available for an Admission Nurse, 4- 10-hour days, will have Wednesday or Thursday off.

PRIMARY PURPOSE OF POSITION: To work collaboratively to admit identified patients to hospice services. The Hospice Admission Nurse will be an expert in the admission process and be well versed in hospice eligibility. The Hospice Admission Nurse will assist with Hospice Talks, providing information about the hospice benefit for potential hospice patients and their families. The Hospice Admission Nurse will make every effort to convert Hospice Talks to Hospice Admissions, based on patient eligibility and acceptance of the hospice philosophy and goals of care.

The goal is to maximize the patient’s quality of life through supportive interventions, medications, and interdisciplinary services, under the direction of a physician’s plan of care. The Hospice Admission Nurse must be fluent in hospice eligibility criteria, have excellent communication and clinical skills, be able to practice independently, collaborate with the IDT and serve patients at all levels of care.

When the home hospice program does not need the Hospice Admission Nurse for new Starts of Care, the Hospice Admission Nurse will assist with revisits, comprehensive visits, prn visits for symptom management, recertification visits, or provide back up support to the Hospice House.

EDUCATION: Graduate of professional school of nursing with state and regional accreditation required; NLN accreditation required; Baccalaureate degree in nursing preferred.

EXPERIENCE: Two to five years of recent hospice nursing experience required.

LICENSURE AND CERTIFICATION: Current R.N. license in the State of Maine. Certification in Hospice and Palliative Nursing encouraged. Current CPR Card. Certification in CAPC Dementia within six months of hire.
