Adecco Healthcare is searching for a Registered Nurse (Case Manager Experience) to become part of our client's team in Long Beach, CA . This position is direct-hire.

$86,000-$90,000

Monday-Friday
8am-5pm
No on-call
No weekends
Hybrid Role (more virtual than field)
May need to drive to OC-LA area (mileage reimbursement provided)
Job Summary
The Registered Nurse (Case Management) role reports to the Chief Medical Officer and is primarily focused on virtual work. Key responsibilities include supporting census management, reviewing patient data across inpatient, ER, and skilled nursing facility stays, outreaching to hospital case managers, coordinating transitions of care and post-discharge planning with healthcare providers, and potentially conducting patient outreach by phone. The RN will also be involved in new patient engagement and overall care coordination. The role involves identifying high-risk individuals who need to be enrolled in the program. The RN will work closely with the wider healthcare team to actively engage these patients.
Qualifications
CA RN license.
BSN degree.
At least 5 years RN experience.
At least 3 years of care management experience in the health plan, home health, and/or hospice.
Case Management experience on the provider or payor side with a focus on patient interaction.
Passion for helping underserved population.
