Maxim Healthcare Services Registered Nurse (RN) - Home Care | Longmont in Longmont, Colorado

Maxim Healthcare is seeking a compassionate and experienced Registered Nurse (RN) to join our home health care team in Longmont. You will provide high-quality care to pediatric and/or adult patients in their own homes, making a positive impact on their lives.

    Responsibilities:

  • Implement and evaluate individualized care plans for contract care patients, considering their unique needs and goals.

  • Collaborates with healthcare providers to regularly review and adjust care plans to address evolving patient needs and ensure optimal outcomes.

  • Provides comprehensive care that includes therapeutic, preventive, and rehabilitative interventions to promote patient recovery and well-being.

  • Monitors patient's condition closely and promptly reports any significant changes to the appropriate healthcare team members.

  • Ensures accurate and timely medication administration, monitoring treatment effectiveness and documenting outcomes.

  • Continuously assesses care plans and identifies opportunities for patient education, care coordination, goal setting, and discharge planning to support optimal patient outcomes.

    Requirements:

  • Current Registered Nurse (RN) license in the state of Colorado.

  • Current Health Certificate (as required by facility or state regulations).

  • Current PPD or chest X-ray.

  • Current BLS certification.

  • At least one year of prior RN experience preferred.
