Confidential Housing Case Manager in LONGVIEW, Washington

SUMMARY: Provides case management and coaching services to help households receiving Homeless Prevention and Rehousing assistance achieve housing stability.  The focus of the case management is to identify the risk factors in the household leading to housing instability, identify the resources needed to eliminate or mitigate those factors, coach the household through developing its own plan of action to achieve housing stability, coach the household as it follows-through with its housing stability plan, and actively help the household access the community resources needed.* * This position monitors client's progress and needs while in the program, links participants to resources, may coordinate resource services, and tracks participants who have exited the program. Though this position will be based out of Lower Columbia CAP, but work will at times be conducted at the household's home, out of the office to help the household access the community resource needed.  The Housing Team Case Manager will also manage weekly schedule and work with community homeless service providers to provide limited after-hours availability.

DUTIES AND RESPONSIBILITIES:

  • Provide case management services to help program participants achieve housing stability. Case management services include but are not limited to screening, intake, assessment of housing stability risk factors, housing stability plan development, coaching, monitoring, linkage to appropriate community resources, follow up, and tracking.

``` - Assist participants in navigating various systems to obtain benefits and resources; which may include completing benefits applications (including SOAR), and facilitation with landlords and housing providers.

``` - Maintain thorough and complete record and case-file documentation of participant's activities and progress while in the program.

``` - Establish and maintain collaborative working relationships with homeless shelters, community resources, and property management companies/ property owners.

``` - Prepare and submit reports to supervisor on a timely manner basis.

``` - Prepare required program reports for funders.

``` - Enter participant's information into the HMIS database in a timely manner and update as needed.

Benefits on the 1st day of month following 30 days of full-time employment, 13 paid holidays per year, 96 hours of vacation and 12 days of sick time in the 1st year of employment.
