Senior Citizens Council of Madison County Lead Case Manager in Madison, Florida

LEAD CASE MANAGER Overview: We are seeking a dedicated and compassionate Lead Case Manager to join our team. Responsibilities: This position manages the case management department, receives referrals, completes initial assessments and annual assessments. Develops care plan in conjunction w/the client, caregiver, and/or designee. Arranges for and follows up on services provided and reviews care plans w/other professionals involved w/service provision. Make home visits. Arranges respite care for caregivers as needed. Coordinate/oversee medical supplies for the agency. Maintains individual case records and organized case file on each consumer w/all required information required by center's policy and procedures and mandated by Area Agency on Aging (AAA) and Department of Elder Affairs (DOEA) for contract compliance, monthly report processing and billing. Advocate for the rights and needs of the elderly and maintain a close relationship with civic organizations, church groups, home health agencies, hospitals, consumers, etc. Supervise Direct Service Workers (DSW) as in-home workers in various programs. Participate in interviewing, hiring, and training of DSW's. Review and approve timesheets and any pre- approved travel reimbursements. Complete ongoing scheduling of in-home workers. Designated APS investigator: each complaint of alleged abuse, neglect or exploitation accepted by the hotline is phoned to the designated Adult Protective Services (APS) investigator in the respective district for contact and action. Requirements: - Bachelor's degree in social work, sociology, psychology, nursing gerontology, or related social service field; OR year for year related job experience or any combination of education and related experience may substitute for a bachelor's degree upon approval of the Area Agency on Aging (AAA) - Proven experience in case management, social work, or related field - Excellent communication skills, both written and verbal - Must be technological savvy - Ability to work independently and as part of a team - Strong organizational skills with the ability to manage multiple cases simultaneously - Must have a valid driver's license and acceptable driving record - Must pass a level 2 background and drug screening. - Additional duties: fulfilling any other necessary responsibilities as required and assigned by the Executive Director per company policies and procedures.
