AdventHealth Part Time Family or Internal Medicine Physician Opening in Manchester, Kentucky
Location : Manchester, KY Specialty: Family or Internal Medicine
AdventHealth Manchester is looking for a dynamic and energetic Physician to join our Skilled Nursing Facility provider team in beautiful Manchester, Kentucky (about 90 minutes southeast of Lexington).
The ideal candidate will be/have:
Provide services within scope of a family or internal medicine physician; geriatric / SNF experience preferred
Examine patients, take medical histories, maintain patient records, identify health risk factors, prescribe medications, make referrals, and otherwise treat health conditions
Provides direct patient care within a skilled nursing facility setting in conjunction with APRNS who manage acute visits and assist with chronic visits. Timely physician-required visits to be done per CMS guidelines
Focus on patient wellness and disease prevention, prevention of hospital readmissions, antibiotic stewardship, advance care planning, collaboration with families and rehab service providers.
Provide health education services and/or counsel patients on how to make healthy lifestyle choices
Risk Management: understand importance of documenting and addressing concerns such as risk of falls, malnutrition, polypharmacy, and pressure injury.
Understands and complies with the Environment of Care/Infection Control Responsibilities
Is knowledgeable and complies with the State’s Institutional Activities and Expectation
Assists medical team with coordination of clinical activities and orders
Licensure as a Medical Provider in the Commonwealth of Kentucky
