This position provides medication management services using evidence-based practices that support client treatment, and facilitate safe, secure and therapeutic interventions.

Primary Responsibilities:

  • Managing weekly schedule per employment agreement re: maintaining scheduled client hours.

  • Performing initial assessment sessions and using a variety of methods to evaluate client needs.

  • Performing follow up sessions and follow jointly development medication treatment plan.

  • Understanding and using medical decision making to bill E/M billing codes.

  • Providing therapeutic interventions to support medication management.

  • Coordinating caseload in a way that aligns with prescription refills and client-centered care.

  • Performing prescription management and following ethical and legal policies and standards for prescribing.

  • Completing prior authorizations for medications, as needed.

  • Completing client-requested documentation related to medications.

  • Referring clients for additional testing as needed to inform medication management.

  • Completing clinically necessary documentation and accurately maintaining clinical records.

  • Coordinating care and attending weekly case consultation.

  • Providing coverage for client care (prescription refills, questions, urgent needs) for other CARE providers when they are out of the office

Required Qualifications:

  • Master's or Doctoral Degree in Nursing

  • APRN

  • Certifications/Licenses Maintain an active, unrestricted license to practice medicine in the State of Minnesota (CNP, APRN)

  • PMHNP-BC or in the process of pursing a mental health certification (e.g., PMHNP-BC) recognized by the board of nursing

  • 1+ years of clinical experience as a APRN in a Psychiatric setting

  • Proven knowledge of regulatory standards governing care delivery and operations

  • Proven basic understanding of Microsoft products (Word, Excel, PowerPoint, etc.)

  • Reside in Minnesota

Preferred Qualifications:

  • Diagnostic assessment experience

  • Experience in outpatient mental health

  • Experience in clinical documentation

  • Experiencein treatment planning and maintaining a caseload

  • Experience entering pertinent information in Electronic Health Records (EHR) Systems

  • Experience reading and interpreting information, such as reports, general correspondence, professional publications, and government regulations, with the ability to share with others as needed

  • Experience reading, interpreting and applying/modeling general information from policy and procedure manuals and other compliance related information

  • Experience presenting information and responding to questions from internal and external stakeholders which can or may include community providers, medical professionals, management, peers, family members and clients

  • Knowledge of the effects of mental illness and seeks information from a wide variety of disciplines applicable to the treatment of persons with mental illness
