Behavioral Health Case Manager
Center Street Community Health Center 136 W Center Street, Marion 43302 Assist in the planning, development, and delivery of outcome-effective treatment. Maintains an ongoing working knowledge of agencies and community/emergency resources for child, adolescent, adult and geriatric clients to advocate for patients. Works directly with assigned patients, assess to determine needs and identified goals. *Bachelor's degree in social work or related field of study preferred associate degree in social work or related field of study and relevant work experience required. *Must be 18 years of age.
