Southwest Mississippi Regional Medical Center Lic Social Worker Case Management in McComb, Mississippi

The Social Worker (SW) will provide psychosocial services to the patient/family with resources, information and services to benefit their adjustment to illness, hospitalization and recovery throughout the continuum of care. The services/resources utilized will be based on the patient's age. The SW will provide timely resource information and assistance. Confidentiality of all information will be maintained. The SW helps patient/family with timely discharge planning, including home health, hospice, swing bed, long term nursing facility placements, LTACH, etc. Provides support to patient/family coping with new diagnosis, ventilator dependency, long term care decision making, psychiatric, cognitive and behavioral issues. SW will work collaboratively with the nurse case manager with patient/family issues & complex discharge disposition. The SW will assist/intervene with ethical or legal concerns, mental health issues, patient, family, team conflicts related to medical plan of care, guardianship, abuse, (domestic violence, child/elder abuse or neglect, rape, sexual assault, guardianship, end of life issues). The SW completes necessary paperwork to assist with arrangements for home health, long term care, etc. The SW will maintain a productive working relationship with community agencies, nursing homes, other resources as well as co-workers. The SW will assist nurse case manager with homelessness/housing issues, financial issues, and crisis intervention. The SW will actively participate in multidisciplinary meetings The SW will notify Director of unusual, complex or potential risk management cases. The SW will encourage patients/family to participate in the medical plan of care & treatment modality. The SW will assist with case manager functions (see case manager job description) when necessary, as well as participate in weekend and on call responsibilities. The SW will provide initial psychosocial assessment when necessary. The SW will evaluate for appropriateness of services based on needs, support systems and available resources. The SW will maintain documentation and accounts for services delivered with department and hospital productivity statistics. The SW will adhere to the NASW (National Association of Social Workers) Code of Ethics. The SW will actively participate in hospital, department in-services when requested. The SW will actively participate in self-learning opportunities to increase knowledge base of current social work issues and/or case management concepts.
