Health First Ambulatory Care Clinical Pharmacist - Anticoagulation Management Services in Melbourne, Florida

POSITION SUMMARY:

To be fully engaged in providing Quality/No Harm, Customer Experience, and Stewardship by: Performing clinical pharmacy services and ensuring that the pharmacy provides optimal clinical services on a daily basis. The Ambulatory Care Clinical Pharmacist assists with the development, implementation, and maintenance of systems that promote desirable patient outcomes through the use of safe, cost- effective drug therapies.

PRIMARY ACCOUNTABILITIES:

Quality/No Harm:

  • Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose.
  • Reads, extracts, and interprets information in patient records, as appropriate.
  • Prepares and presents drug use evaluation (DUE), drug utilization review (DUR), and pharmacy performance measures summary to P&T committee.
  • Reviews the use of targeted medications for compliance with current pharmcoeconomic programs.
  • Assists the Clinical Coordinator in the development, implementation, and maintenance of clinical pharmacy services.
  • Facilitates the development and implementation of treatment guidelines, protocols, formulary changes, and critical pathways as approved by appropriate committees.
  • Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, adverse drug reactions, and appropriateness of drug and dose.
  • Maintains current pharmacist licensure.
  • Completes all competence/skills assessment requirements (see attached competence assessment/skills list).
  • Maintains patient and department confidentiality.
  • Participates in the quality improvement activities of the department. Collects and analyzes data and maintains documentation of all clinical activities.

Customer Experience:

  • Provides accurate, adequate, and timely drug information to the professional staff and patients.
  • Provides training and education to pharmacists, residents and students regarding pharmaceutical care and pharmacoeconomics.
  • Supervises, instructs and evaluates residents and pharmacy students.
  • Participates in community educational programs (e.g., diabetes classes, health fairs).
  • Assists Clinical Coordinator with policies and procedures for clinical activities (e.g., establishing prior authorization criteria).
  • Establishes productive, collaborative relationships with staff members and within the community.
  • Attends assigned committee meetings and reads pertinent email communications.
  • Sustains professional competence and promotes development through participation in in- services, workshops, seminars, and professional organizations.
  • Presents one major clinical presentation annually (ex. FSHP, AMCP, ASHP, CME, etc)

Stewardship:

  • Facilitates the identification, resolution, implementation, and communication of cost-effective medication use issues in collaboration with the medical staff.
  • Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost effective therapeutic alternatives to medical staff as needed.
  • Reads and evaluates the medical literature for studies supporting the pharmaceutical care and pharmacoeconomic programs.
  • Prepares reports on pharmaceutical care and pharmacoeconomic activities including financial impact, as directed.

QUALIFICATIONS REQUIRED:

  • Graduated from accredited college of pharmacy
  • Must possess a Pharm.D. degree
  • Graduated from ASHP accredited PGY1 residency program or equivalent clinical experience required; PGY2 preferred
  • Must have a license to practice in the state of Florida by Board of Pharmacy
  • Basic Life Safety with CPR certification required
  • Hours may be long and irregular; includes evenings, weekends, and holidays as necessary

PHYSICAL DEMANDS:

  • Repetitive use of hands and fingers. Good hand and wrist dexterity to enable typing and/or computer work for 8-10 hours/day.
  • Good eyesight to read literature, reports, and clinical documentation.
  • Good hearing to understand phone conversations, customers and those in which the position depends.
  • Ability to work in noisy distracting atmosphere, to handle frequent interruptions, to adapt to changes in workload and work schedule.
  • Handle large volumes of work for long periods of time.
  • Walk, bend, stoop, twist, sit, stand for long periods of time, 8-10 hours a day.
  • May require lifting and carrying light loads (Up to 40 lbs.)
