Summary The Clinical Pharmacy Specialist assesses and coordinates compliance activities with all Pharmacy directives, handbooks, policies, laws, rules, and regulatory reviews in both the inpatient and outpatient settings, including all research activities. Provides expertise related to medication safety, develops, conducts and reports quality assurance monitors for medication safety. Responsibilities Serves as lead for medication error reporting for pharmacy services and assures proper review and reporting through P&T. Serves as the main pharmacy point of contact (POC) for Joint Patient Safety Reporting (JPSR) event reports, investigates and provides reports back to Pharmacy leadership. Works with the Drug Safety Committee to reviews and responds as appropriate to safety notices such as ISMP, PBM Safety Bulletins and Drug Recalls; completes assessments related to medication safety and quality of care. Works with Patient Safety and Quality Management to perform facility medication aggregate reviews to identify opportunities for medication management and medication safety processes improvement. Analyzes data, identifies trends, prepares and/or reviews reports involving trend analyses. Provides medication safety training, knowledge and skills to medical staff, nursing staff pharmacy staff and allied health personnel, students and trainees as needed. Works with the Controlled Substance Coordinator to ensure compliance with all controlled substance directives. Works with the Clinical Research Pharmacist and Clinical Staff Pharmacists to ensure compliance with all research protocols. Develops dispensing procedures for all active studies in a uniform format containing concise protocol summaries, dispensing instructions, lists of authorized prescriber, dispensing records, and other information needed by pharmacists. Responsible for assessing and coordinating compliance activities with all Pharmacy directives, handbooks, policies, laws, rules, and regulatory reviews (OIG, TJC, LTCI) in both the inpatient and outpatient settings to include all research activities. Completes the clinical review and analysis of all Adverse Drug Events (ADEs) (i.e., adverse drug reactions and medication errors) associated with the medication management process. Provides results of reviews and evaluations in a concise, usable manner. Develops, conducts and reports quality assurance monitors for medication safety. Develops and coordinates the Pharmacy Quality Improvement program. Serves on hospital or departmental committees as requested. Works with pharmacy leadership to ensure regulatory and compliance in each section are consistently attained to meet accrediting bodies standards. Presents and documents all in-service education programs to pharmacy staff and other health professionals related to medication safety, and how to manage the quality improvement program for the pharmacy service. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (Transcripts Required) OR Graduate of a foreign pharmacy degree program if you are able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). (Documentation Required) Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. Grade Determinations: May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grandfathering Provision. All licensed pharmacists employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure/certification/registration that are part of the basic requirements of the occupation. For employees who do not meet all the basic requirements required in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply Employees grandfathered into the GS-660 occupational series may be reassigned, promoted up to and including the full performance (journey) level, or changed to lower grade within the occupation, but may not be promoted beyond the journey level or placed in supervisory or managerial positions. Employees who are appointed on a temporary basis prior to the effective date of the qualification standard may not have their temporary appointment extended or be reappointed, on a temporary or permanent basis, until they fully meet the basic requirements of the standard. Employees initially grandfathered into this occupation, who subsequently obtain additional education and/or licensure/certification/registration, that meet all the basic requirements of this qualification standard must maintain the required credentials as a condition of employment in the occupation. If a licensed pharmacist who was retained under this provision leaves the occupation, the employee loses protected status and must meet the full VA qualification standard requirements in effect at the time of reentry to the occupation. Grade Determinations: Specialized Experience, GS-13: In addition to the GS-13 requirements, must have 1 year of experience equivalent to the next lower grade level, GS 12. which is directly related to the Clinical Pharmacy Specialist position this experience may include, but not be limited to: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy; AND In addition, demonstrate the following knowledge, skills, and abilities (KSAs)? Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
