Minnesota Visiting Nurse Agency Clinical Documentation Improvement Supervisor in Minneapolis, Minnesota

_SUMMARY_

We are currently seeking a CDI Supervisor t* *to join our Clinical Documentation Improvement team. This full-time role will primarily work remotely(days).

Purpose of this position:First line supervisory position chiefly responsible for supervising inpatient Clinical Documentation Specialists who are responsible for active concurrent review, feedback, and education to improve documentation of all conditions and treatments in the inpatient medical record, to ensure an accurate reflection of the patient condition in the associated MS-DRG assignments, case-mix index, severity of illness and risk of mortality profiling and reimbursement. Position will require considerable knowledge, experience, judgment and ability to resolve problem related to ICD-9 and ICD-10 documentation requirements, overall documentation requirements within HCMC, job responsibilities of clinical documentation specialist, inpatient coding and billing, working closely with inpatient coding and billing to resolve documentation issues, PSI/HAC reporting, proving leadership and direction to clinical documentation specialists through reviews and feedback.

_RESPONSIBILITIES_ * Participates in the selection, training, and evaluation of staff engaged in the performance of Clinical Documentation Specialists and CDI Quality Coordinator * Reviews Clinical Documentation Specialists work for accuracy and conformance with departmental policies and procedures; tracks and monitors productivity goals (charts/hour, query rate) and standards. Reviews results with manager and Clinical Documentation Specialists on monthly basis or more frequently if needed * Assists in development of long range plans for work teams’ activities and monitor results to ensure compliance with expectations * Determines priorities, schedules and assigns work as required to Clinical Documentation Specialists and CDI Quality Coordinator * Monitors the quality and quantity of work produced by the Clinical Documentation Specialist and gives feedback on review of work, queries, DRG assignment on a quarterly basis * Assists in developing and maintaining all policies and procedures pertaining to the Clinical Documentation Improvement Program and the Clinical Documentation Specialties duties * Conducts audits of post discharge charts prior to billing to ensure accurate diagnoses and DRG assignment. This is coordinated with HIM coding and HIM Auditors along with Physician Liaisons * Assists with reports as instructed * Serves as a resource for coding and Clinical Documentation questions * Assists in the training and education of new Clinical Documentation Specialists * Collaborates with other departments as needed for documentation quality * Oversees PSI/HAC reviews done by CDI quality coordinator * Coordinates the escalation of documentation queries and issues to Physician Liaison’s as needed * Works with Clinical Department physician chairman to obtain timely completion of queries and provide physician education * Assists in development and planning of education of providers about quality documentation, ICD coding, profiling and hospital metrics * Collaborates with the inpatient coding supervisor to resolve coding documentation issues and DRG miss matches * Performs other duties as assigned, but only after appropriate training

_QUALIFICATIONS_ /Minimum Qualifications:/ * Associate degree in Nursing, Health Information Management or other healthcare related field * Prior supervisory or leadership experience * Three to five years of experience working with documentation and/or ICD coding to meet quality, financial and regulatory requirements

  • An approved equivalent combination of education and experience

-AND-

  • Certified Clinical Documentation Specialist (CCDS) or eligible

-AND-

  • Maintain current licensure by the Minnesota Board of Nursing as a Registered Nurse (RN)

-OR-

  • Maintain current certification as a Registered Health Information administrator or Registered Health Information Technician /Preferred Qualifications:/
  • Bachelor’s Degree in Nursing, Health Information Management or other healthcare field /Knowledge/ Skills/ Abilities:/
  • Proficiency with computer hardware (PC, laptop, etc.) and software (Microsoft Office, Epic Electronic Medical Record, 3M Doc-MS and reporting systems, etc.), as well as the ability to learn/utilize programs on an ongoing basis
  • Self-directed and self-motivated to work both independently and in a team atmosphere
  • Contributes to process improvement activities and is adaptive to changing and adjusting workflow to meet the needs of the customer and the program
  • Demonstrates the ability to utilize individual and team data metrics to analyze workflow to allow self/team adjustment of priorities and influence process improvement suggestions
  • Demonstrated proficiency in the ability and desire to develop positive working relationships with physicians and other professional health care staff
  • Excellent consultative skills (i.e. strong interpersonal and negotiation skills, ability to develop strong, trusting working relationships, in order to gain support and achieve desired results)
  • Demonstrated ability to offer innovative and thoughtful ideas for process improvement
  • Knowledge of care delivery documentation system and related medical record documentation
  • Knowledge of age-specific needs and elements of disease process and related procedures
  • Strong broad-based clinical knowledge and understanding of pathology/physiology
  • Excellent written and verbal communication skills and critical thinking skills
  • Working knowledge of inpatient admission criteria
  • Working knowledge of overall documentation requirements
  • Excellent data analysis and process evaluation skills
  • Working knowledge of Medicare reimbursement system and coding structures
  • Skilled in overseeing the efforts of individuals within the department
  • Ability to effectively involve department staff in the application of quality measurement and improvement tools
  • Skilled in fostering a positive workplace culture and building inclusive workplace teams
  • Demonstrated ability to quickly learn new technologies and systems and provide instruction on complex processes
  • Capable of assisting managers with decisions and activities
  • Ability to appropriately prioritize and carry-out work
  • Ability to comfortably and effectively facilitate meetings and feedback sessions
