Access: Supports for Living Supported Housing Case Manager in Monticello, New York

Overview: Access: Supports for Living is seeking Supported Housing Case Managers in Monticello, NY.

Supported Housing Program provides housing for chronically homeless single adults who are eligible for SSI or SSD, and suffer from Serious and Persistent Mental Illness (SPMI) or are diagnosed as Mentally Ill and Chemically Addicted (MICA). It also provides housing for SPMI individuals who reside in OMH psychiatric centers, Article 28 inpatient hospitals, residential treatment programs and adult homes as well as housing for individuals who are SPMI and have been residents of either OMH Psychiatric Centers or OMH operated residential programs for one year or longer.

The Supported Housing Case Manager will:

Engage the recipients and create a shared Housing Health and Recovery Plan that will reflect the principles of person-centered, strength-based recovery, with the ultimate goal of self-sufficiency. Provide outreach and assistance securing apartments in the recipients desired community. Provide services that will touch all of the life domain areas including employment, finance, permanent relationships, medical needs, health, legal concerns, family, education, transportation, safety, culture, spirituality, and social. Partner with recipients as they integrate into the community of their choice and support them to make informed choices of the available supports, services, and recreational opportunities. Assist with a plan for discharge when the recipient is ready to transition out of Supported Housing. Anticipate supports and services needed to highly support the transition and ensure implementation of all linkages to facilitate success. Serves as a model, guide and bridge towards highest level of independent community living. Skills: engagement, assessment, crisis intervention, linking, community integration Comply with necessary paperwork regulations Comply with regulations set by employer, as well as NYS OMH Work closely with community providers on facilitating a smooth transition for all referrals into the Supported Housing Program. Share responsibility for off-hours and emergency situation

Education: Bachelors Degree in Health and Human Services or related field OR A Credentialed Alcoholism and Substance Abuse Counselor/ CASAC ALL POSITIONS ARE SUBJECT TO A CRIMINAL BACKGROUND, FINGERPRINTING AND MOTOR VEHICLE REPORT CHECK.

Major Skills Required to perform the job: Computer Skills Organizational Skills Empathy Excellent interpersonal and engagement skills

Lived Experience Preferred

Requirements: Reference Check Valid NY State Driver License Own Car 2 years of experience working in case management or mental health services and/or equivalent experience ALL POSITIONS ARE SUBJECT TO A CRIMINAL BACKGROUND, FINGERPRINTING AND MOTOR VEHICLE REPORT CHECK.

Shift, Salary, and Benefits: This is a Full-Time position 40 hours a week, First Shift, Monday-Sunday, Rotating Weekends, $19.50 hour, Benefits include: Health Insurance, Dental Insurance, Paid Vacation, Paid Sick Leave, Holidays, and 401K/Retirement.
