BAYADA Home Health Care Licensed Practical Nurse ( LPN ) - Transition Team in New Castle, Delaware
We have immediate opportunities on our Transition Team for PEDIATRIC LPNs interested in helping infants, children, and adults receive complex care at home in New Castle County, Delaware! This team will support the transition of care from the hospital or from our HICU® Health Services team to ongoing home care services. As part of the Transition Team, you will will be responsible for delivering quality care to clients and providing extensive training to the ongoing care team.

Qualifications for Transition Team LPN:
Have a current license as a Licensed Practical Nurse ( LPN )
NICU, PICU, ICU, or home care trach/vent experience required
Achieve a passing score on high-tech Adult and Pediatric simulations.
Hold current CPR certification
Ability to work in a teaching setting to prepare and educate the ongoing BAYADA care team
Availability to commit to an ongoing, consistent schedule
Ability to travel throughout New Castle County
