PAFACOM Inc. Direct Support Professional (DSP) in New Jersey

High level of communication and support to Team Leaders, the Residential Supervisor, consumers, family members, guardians, case managers and members of the IHP/IDT, contributes to the daily operation of the group home and ensures provision of a safe and productive living environment, ensures that order is maintained the home and among the residents, assists/encourages residents to learn new skills/strengthen their current abilities, plans and helps residents purchase and provide nutritious meals and clean-up; and, prepare special diets as required, completes appropriate quality assurance reports/follow-up in a timely and thorough manner, assists residents with their IHP goals and house chores, provides various types of social and recreational activities to residents; and, to encourage the residents interest and participation in whatever leisure activities they choose to pursue, completes designated shift duties within the specified time frames, attends and actively participates in house and staff meetings, and completes any follow-up and/or assignments identified in the meeting, makes safety, staffing or systematic recommendations to the Team Leader as appropriate, actively participates in appropriate in-services/ training as scheduled, ensures timely and accurate completion of clinical documentation required by the state for each consumer in the home, to include: DTR's, Client Notes and the Daily Log, completes Unusual Incident Reports and other required documentation as necessary, secures all consumer information and ensures confidentiality, ensures needs of consumers in the home are being met, researches, obtains and implements program-related materials, cooperates with the Agency and the Division of Developmental Disabilities during any inspection or investigation, provides transportation and escorts residents to their day program, work, doctor/dentist appointments, church services or other destinations they may choose or that may be necessary, performs lifting responsibilities as needed: lifting residents from wheelchairs, chairs, floor, etc; onto toilets, chairs, bed, etc; and, assisting consumers as needed in various transfers, participates in IHP meetings as necessary, identifies when a resident is ill and takes the necessary measures as needed, administers medication and completes required documentation as per state regulations, forms supportive relationships with all residents and promotes their inclusion into the community activities of their choice, cooperates with the agency and with the state in any inspection or investigation, completes all required state and Agency training, both the initial training and subsequent re-certifications or refresher courses, promotes group home safety by taking immediate action in emergency situations, and by alerting the Team Leader of any health or safety issues in a timely manner, successfully completes DDD required training as indicated in the SE/DP manual, complies with the DDD SE/DP manual and applicable state, local and federal regulations, represents the Agency at inter-agency, state and various community forums as assigned, adheres to all safety practices and policies as outlined in the personnel handbook, environmental Management Plan, Infection Control Plan, Training/Residential Policy Manuals, completes all required safety training, as part of safety plan, participates in safety inspections/Safety Committee meetings as assigned by the Safety Committee Chairperson, contributes to the overall safety and well-being of consumers, employees and visitors by keeping all work spaces free from debris, identifying and removing safety hazards and immediately reporting safety hazards and potential risks to the area supervisor and to the Safety Committee Chairperson
