The Hospice Licensed Practical Nurse (LPN) works closely with RN Case Manager(s) to provide holistic patient-centered care, quality care end of life. As part of a collaborative interdisciplinary team, the Hospice LPN provides compassionate supportive care to our Hospice patients, as well as their caregivers, and families.

Highlights for a Hospice Licensed Practical Nurse (LPN):
One on one patient care visits in the home or care facility ALF/LTC
Timely completion of required documentation, adhering to agency policy
Attend and participate in weekly IDT meetings
Manage comfort measures to ensure quality of life that is based upon the patient’s goals
Follow patient care plans, and orders
Coordinate with RN Case Manager, attending providers and other IDT team members
Engage in respectful communication (both verbal and written) with team members, patients, family members, and the community
Adhere to agency wide policies
Other duties as assigned
Qualifications for Homecare Registered Nurse (RN) Case Manager:
Active NH LPN license
Minimum 2-3 years direct care experience
End of life or Hospice experience preferred
Excellent communication skills necessary
Proof of Flu vaccination
Reliable transportation
Valid driver's license
Automobile insurance
Physical exam within the last year
Two-step TB test
