The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. The above hiring range represents the University's good faith and reasonable estimate of the range of possible compensation at the time of posting.

Position Summary

Candidate with a Licensed Master of Social Work (LMSW) is sought for the Department of Neurology Aging and Dementia and the Taub Institute. The candidate will serve as a Care Navigator for the Medicare funded GUIDE- Guiding an Improved Dementia Experience Program.

Responsibilities

The Licensed Master of Social Work (LMSW) will serve as a care navigator for the Medicare funded GUIDE- Guiding an Improved Dementia Experience Program. The GUIDE model is designed to enhance quality of care by improving quality of life for people with dementia and reducing burden and strain on their caregivers.

The LMSW/Care Navigator shall be the primary point of contact for the GUIDE Beneficiaries and their Caregivers. The LMSW/Care Navigator shall be part of the Care Team and shall provide ongoing contact with the GUIDE Beneficiaries and/or Caregivers in order to revise and maintain the person-centered care plan as needed, identify unmet needs and coordinate clinical and community-based services and supports, monitor medication management and adherence, and provide caregiver education and support as may be needed to support the GUIDE Beneficiaries and their Caregivers.

The LMSW/Care Navigator shall complete the initial GUIDE required training and shall participate in CMS-sponsored learning activities designed to drive improvement and foster innovations in care delivery through sharing best practices or new knowledge that emerges from participation in the Model. The candidate shall also serve as a point of contact to receive newsletters and communications from CMS related to the Model.

Specific Duties:

  • Provide social work support for people with cognitive and degenerative disorders of the brain including Alzheimer's disease, Lewy Body Disease, and Frontotemporal Dementia.
  • Complete all training and documentation required as member of the MEDICARE funded GUIDE program.
  • Complete forms required for referrals including homecare, in-home rehab services, respite, transportation, and hospice services, respite care, disability, long term care, FMLA, M11Q and durable equipment forms.
  • Compose letters of medical necessity.
  • Make home visits to GUIDE Beneficiaries annually or as needed.
  • Provide case management, crisis intervention, and supportive counseling services.
  • Provide referrals and resources to patients and caregivers.
  • Participate in staff meetings, in-services, administrative activities, and serves as the point of contact for Medicare communications related to the GUIDE model.
  • Give trainings and presentations regarding social services for individuals with dementing disorders and their caregivers.
  • Works closely with community non-profit organization partners to facilitate services to caregivers.
  • Work with PharmD and NP and LMSW on Guide Care Plan.
  • Document accurately in the patient electronic health record and submit data into the CMS database to fulfill the data reporting requirements of the GUIDE model.
  • Assist with the supervision of our MSW student interns.
  • Perform other related duties and responsibilities as assigned/requested.

Minimum Qualifications

  • Bachelor's degree or equivalent in education and experience, plus three years of related experience.

Preferred Qualifications

  • Must hold a Master's degree in social work.
  • Related experience in social work with the elderly.
  • Proficiency in EPIC.
  • SIFI Certification or SIFI Eligible strongly preferred.

Other Requirements

  • Must hold a New York State LMSW.
  • Certificate from the New York State Education Department.
  • Bilingual English and Spanish.
