    JOB SUMMARY

    The Social Worker is part of a human service team that develops and executes service and care plans for residents to provide a supportive and empowering environment that fosters stabilization, independence and self-reliance, and skills development. The Social Worker plays a key role in assessing resident needs, providing direct services, bridging residents to external service providers, and promoting healthy behaviors and practices amongst residents.

    Pay: $70,000 annually.

    FLSA: Exempt. Not eligible for overtime.

    Schedule: 10am-6pm Sunday-Thursday (off Friday & Saturday).

    Location: Midtown Manhattan, NY

    JOB-RELATED DUTIES – ESSENTIAL FUNCTIONS

    These duties are essential and specific to the successful implementation of this position.

    SERVICE AND CARE COORDINATION

  • Ensure residents receive quality and appropriate services per organizational principles and program requirements.

  • Conduct comprehensive biopsychosocial assessment to identify immediate and long term needs in healthcare, food and nutrition, childcare and youth education, employment and vocation, immigration and citizenship status, English language proficiency, trauma care and recovery, and other social need as disclosed by residents.

  • Coordinate with Case Managers to design service and care plans that correspond to biopsychosocial assessments and address human, health, education and employment, recovery and stability needs.

  • Coordinate workshops and activities that enhance self-care and healthy habits, cultural and social adjustment, recovery and stabilization, and English language acquisition.

  • Collaborate with external providers and organizations to assist residents with social and cultural adaptation, address social and human service needs, access preventive and remedial healthcare, and access to mental health services including trauma recovery and therapeutic counseling.

  • Meet with residents in assigned caseload at least once a week to assess progress with service and care plans. Modify plans as needed.

  • Coordinate with Case Managers to conduct weekly room inspections to ensure healthy hygiene and housekeeping practices.

  • Coordinate with Case Managers on discharge plans to ensure needed service linkages and referrals to community-based organizations are in place.

    INTERVENTION SERVICES

  • Develop and facilitate workshops and activities with external service providers; monitor resident engagement and assess impact of activities and workshops to individual residents and the general resident population.

  • Provide crisis intervention and de-escalation interventions as needed to ensure the safety of residents, staff, and visitors in the building. Follow-up with residents post Priority 1 and 2 incidents and bridge residents to services as needed.

    GENERAL DUTIES

  • Document interactions and communications with and on behalf of residents in CARES and AWARDS.

  • Perform other duties as assigned by supervisor or Program Director.

Qualifications

What do I need to apply?

Education and Experience:Master’s Degree in Social Work, LMSW preferred. Three (3) years of experience in case management, counseling, or direct advocacy for clients in social service settings. Experience with immigrants and immigration issues prioritized.

  • Communication:Excellent verbal and written communication skills. Proficiency in English required, multi-lingual and Spanish proficiency prioritized.

  • Computer skills:Basic competency in MS Word, Excel, Outlook and the internet. Experience with CARES, HOME, CAPS, and AWARDS prioritized.

  • Math Skills:Ability to formulate simple financial budgets.

  • Physical Performance:Ability to make unit visits (walk distances, climb stairs, etc.)

  • Reasoning Ability:Ability to prioritize, make appropriate decisions and judgment calls.

  • Other Skills:

  • Experience conducting biopsychosocial and needs assessment surveys.

  • Experience in service and care plan design or trauma and recovery counseling.

  • Experience working with homeless individuals and people living with health and/or mental health issues and substance use. Ability to engage residents.

  • Experience facilitating workshops and group activities.

  • Ability to work independently and as part of a team.

  • Must be highly organized, be willing to assume responsibility, work well under pressure, meet deadlines, multi-task, detail-oriented, and be flexible.
