Purpose of Position:
Provide medical nutrition therapy; develop and implement nutrition care plans for individuals and groups in outpatient, inpatient and long-term care settings for pediatric, adults and elderly patients. Provide health education to individuals and groups with chronic care needs to assure positive outcomes.

Essential Functions:
- Provide medical nutrition therapy, develop and implement nutrition care plans for individuals and groups in outpatient, inpatient settings and long-term care settings. Provide medication nutrition therapy for all stages of the lifespan. Document in the electronic health record.
- Work with communities to set and achieve community based diabetes program outcomes and be able to document achievements.Collaborate with communities to provide diabetes and prevention activities related to fitness, nutrition and tobacco cessation.
- Create, plan and execute health education activities for diabetes and chronic disease prevention projects. Assist with CAMP initiatives as needed; such as the Nome Health Fair, Summercise, Walks and Runs, Tobacco Cessation events and Health Aid Trainings.Plan and provide educational opportunities for specific groups in the region, including by not limited to Nome Public Schools, Nome Community Center, Boys and Girls Club and Nome Youth Facility.
- Collaborate with all health educators, dietitians, prevention coordinators, tobacco cessation specialists, etc. assigned to the CAMP Program. Collaborate with service providers and professionals as needed.
- Lead, collaborate with, and assit with the monitoring and reporting to satisfy grant requirements.
- Perform other duties as assigned.
