Program Coordinator - Life Skills Education Advancement Program (LEAP) - $20.63/Hr

FT 40 Hr/wk
Provides 24/7 support to young adults in a residential setting while teaching daily living skills in accordance with agency values and procedures. Responsibilities include: implementing policies and procedures, training new employees, charting, medication cueing, case management, crisis intervention, creating and providing member IRPs, assisting Unit Director with assessments, admissions, discharges. Documentation of services in the Electronic Medical Records System (EMRS). CNA/HHA/PCA/PCT preferred.

Sun, 12:00pm-8:00pm, Monday-Thursday, 2:45pm-10:45am
Duties and Responsibilities:
- Implements program and Agency policy and procedures
- Assists in data collection for required reporting and program evaluation
- Assists in developing and monitoring the effectiveness of member service plans
- Assists in training new staff in their roles
- Participates in the service plan process which includes creating initial IRP, crisis planning, risk assessments as designated, and collaborating with Unit Director and Clinician as needed
- Attends team meetings as designated by the Unit Director
- Provides rehabilitation, basic support, case management, and crisis intervention per program participant service plans
- Provides program admission as designated by Unit Director
- Provides required documentation of services within the deadlines of the Agency
- Reports on residential status to Program Director and staff
- Provides input on team decisions regarding admission, discharge, and counselor assignments
- Assists program participants in maintaining apartments and landlord standards, when applicable
- Assists program participants in financial matters as needed
- Provides transportation related to services when necessary
- Maintains CPR/FA/AED certification
- Monitors program participant compliance to their conditional release order when applicable
- Provides coverage as needed at any of the Agency sites per Agency need
- Engages in continuous concerted efforts to access, obtain, and respond to pertinent Agency information/communications, and disseminates as appropriate
- Provides conditions that maximize safety for co-workers and program participants
- Other responsibilities of Reliance Health staff, includes participation in Agency events, adhering to Agency Policies, tr ining requirements etc.
