Direct Support Professional - Continuous Residential Support Program - $18.54/Hr

Closing Date: 12/30/2024

FT 40 Hr/wk

Provides 24/7 residential support and individualized program support to program participants at their level of need in a secure, environment promoting self-esteem, and self-sufficiency, while helping them grow and achieve maximum levels of autonomy and high quality of life. Responsibilities include: accurate documentation for services and required supervision/line-of-sight support requirements, assistance with budgeting, employment and educational counseling, health and wellness, medical services including medication management, transportation, and ADL skill development (examples include cooking, grocery shopping, menu planning, and laundry).

Tuesday - Saturday, 3:00 pm - 11:00 pm

Duties and Responsibilities:

  • Provides residential and day program services to adults with intellectual disability in a manner consistent with each program members level of need assessment and individual plan
  • Provides support and supervision to each program member in accord with positive behavior support plans and/or crisis intervention and prevention plans
  • Maintains 1:1 line of sight supervision in the community for program members who require this level of support
  • Maintain an awareness of each program members medical and mental health status while on shift and report changes of status to the nurse and the residential program director
  • Accompanies program members to medical and mental health related appointments to help ensure an understanding of treatment plans, an accurate and comprehensive documentation record and timely follow throughs on provider recommendations
  • Provides medication cueing support in a manner consistent with DDS Programs Policy and each program members needs
  • Implements DDS Programs documentation practices utilizing Therap including daily ISP documentation, TLOGS, health tracking and time tracking when applicable
  • Assists program members with completing the basic requirements of daily living including meal planning, grocery shopping, cooking, cleaning, laundry, housekeeping, transportation or other as needed
  • Assists Residential Program Directors and Program Coordinators with maintaining the home environment to best practice agency safety standards and DDS quality service safety requirements for CRS homes
  • Assists program members with locating, planning, and articipating in community based social and recreational events

Education and Experience:

  • High School Diploma or G.E.D
  • Valid CT Drivers License, vehicle registration and insurance
  • Good Driving record
  • Bi-lingual preferred
