If you reside in the Greater St Louis Area, MO, you will have the flexibility to work remotely* as you take on some tough challenges.
Primary Responsibilities:
Providing evidenced based trimester specific and postpartum education to promote a healthy pregnancy, delivery, and postpartum outcomes for mother and baby
Coordinating care for high-risk maternity members
Proactively engages the member to manage their own health and healthcare
Making outbound calls to assess members’ current health status
Identifying gaps or barriers in treatment plans
Educating members on disease processes
Encouraging members to make healthy lifestyle changes
Documenting and tracking findings
Interacting with Medical Directors, providers, internal and external partners on challenging cases
Creating a positive experience and relationship with the member and your team
Performing outreach to providers, hospitals, and clinics
Making referrals and connecting members to community resources
Coordinating services as needed (home health, DME, etc.)
Attending community events
Able and willing to travel within the assigned region per the business needs
Performs all other duties assigned

Required Qualifications:
Current and unrestricted Missouri RN license
3+ years of OB or NICU clinical nursing experience
2+ years of experience working in Medicaid
2+ years of experience in a hospital setting, acute care, direct care experience as an RN and / or experience as a telephonic Case Manager for an insurance company
Proficiency with Microsoft Office Suite, with the ability to navigate a Windows environment
Computer / typing proficiency to enter / retrieve data in electronic clinical records; experience with email, Internet research, use of online calendars and other softwareapplications
Willing or ability to travel up to 50% of time in assigned region to visit Medicaid members in their homes and / or other settings, including community centers, hospitals, or providers’ offices
Reside in the state of Missouri
Preferred Qualifications:
Certified Case Manager (CCM)
Certified Lactation Consultant (CLC)
Experience working with high-risk maternity population
A background in managed care
Soft Skills:
Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying information in a manner that others can understand, as well as ability to understand and interpret information from others
Excellent organizational and time management skills
Ability to work in a fast-paced environment
Ability to work independently and as a team and maintain good judgment and accountability
Ability to multi-task and prioritize tasks to meet all deadlines
Work collaboratively with various groups to achieve specific team goals

The hourly range for this role is $28.61 to $56.06 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
