Overview of the role:
This role works supports our teams in the H.O.P.E. program. The Home Options Path to Empowerment (H.O.P.E) Program managed through AgeOptions helps individuals transition from a long-term care facility and move into their own home in the community. Utilizing interdisciplinary teams of social workers, community care partners, nurses, occupational therapists and social services professionals to assist members to transition to community living. The Program Support Assistant provides administrative support to team members including—updating case notes and documents in electronic case management systems, creating reports, monitoring completion of deliverables and responding to client’s needs and requests. This role may work periodically in the field directly assisting clients.
What are the Specifics:
Maintaining electronic case management databases
Organizing data for reporting purposes
Coordinating correspondence and data requests
Creating reports for staff, supervisors and funders
Ordering and managing supplies and durable medical equipment
Coordinating client services and facilitating payment
Monitoring external partners’ contractual compliance
Other projects as needed related to the grant and team.
Required Experience:
Who are we looking for:
A highly organized social services professional who likes the administrative details and help the team meet and exceed goals
Someone who gravitates toward the details and has a high proficiency to prioritize, meet deadlines while considering diverse perspectives.
Has a good handle on technology and has a desire and ability to master and navigate different systems and platforms. Examples like--Microsoft Suite, Database platforms, and Zoom
A flexible service driven individual who can easily move from running a report to training a group of colleagues to servicing a client in need
A communicator who can learn complex information and translate it into easily understandable language
A team oriented worker who has at least 1 year of professional office experience or business skills certification/education. Additionally, at least 1 year of customer service or customer facing experience.
