SEAGER MEMORIAL CLINIC Community Health Worker in OGDEN, Utah

Job Summary:

The Community Health Worker (CHW) will further the Seager Memorial Clinic's mission of providing free, easy-access, non-judgmental, charitable medical, dental, vision and mental health care to the underserved, by supporting clinic operations and connecting patients with community resources. The CHW builds trust with underserved communities and seeks to understand and minimize barriers to accessing needed services. 

Responsibilities:

Clinic Operations:

- - Assist patients in scheduling appointments for diagnostic testing and specialty care.

- Follow and manage external medical referrals and obtain records & results. - Monitor incoming and outgoing referrals on the Unite Us platform.  - Connect patients with local providers of community resources and services.  - Respond to phone and email inquiries from patients about clinic services.  - Oversee special projects as identified by the Executive Director and/or Clinic Manager.

COVID Support:

- - Promote COVID testing and vaccination resources to patients.

- Monitor COVID supplies and restock as needed.

Community Outreach: 

- - Attend weekly Reentry Fair and other community tabling events.

- Represent the Seager Memorial Clinic at monthly Weber County Community Reentry Coalition meetings.

Ongoing Development:

- - Participate in a monthly meeting at the Weber-Morgan Health Department to learn from and collaborate with other CHWs in the organization's service area. 

- Complete CHW State Certificate online training through the Utah Community Health Workers Association.

Ongoing Reporting:

- - Complete monthly reports summarizing outcomes as directed by the Executive Director.

Required Education:

  • High school diploma

Skills & Experience:

  • Bilingual Spanish (strongly preferred)

  • Ability to travel throughout the organization's service area

  • Strong communication skills

  • Strong organizational skills

  • Ability to identify problems and find solutions

  • Experience working with diverse communities

  • Experience in community outreach and education

  • Experience using Gmail, Google Drive, Google Meet, and Zoom

  • Experience using social media platforms

  • Represent the Seager Memorial Clinic at monthly Weber-Morgan Local Homeless Council meetings on an as needed basis. 

  • Promote Seager Clinic at special events in the community.

  • Promote Seager Clinic at various tabling events in the community.

  • Promote Seager Clinic on social media platforms (Facebook, Instagram, Linkedin, and X).
