In this role you'll be responsibile for adequately assessing our residents' needs and creating personalized care plans based on the assessments. Promote cross-functional communication to ensure care plans are followed. Ensure appropriate reimbursement for resident care services. Be well versed with current federal, state, and local standards, guidelines and regulations.
