  • Coordinates patient records for current lung cancer program. Assists with ongoing patient participation requirements.

  • Prior experience utilizing computer applications for word processing, accessing databases, internet research, and emailing required.

  • Essential Functions I

  • Prepares and completes data submission appropriately as required by American College of Radiology (ACR) protocols.

  • Prepares and completes data submission forms as required.

  • Retrieves data appropriately from source documents for data entry.

  • Verifies source documentation matches data submitted.

  • Queries are answered in an accurate and timely manner.

  • Follow up forms to be submitted monthly.

  • Review reports from ACR to ensure that data submission is current and accurate.

  • Retrieves, assembles, and maintains patient records to meet ACR/Screening standards.

  • Evaluates new patient referrals for eligibility for screening according to current national health guidelines.

  • Is Familiar with and follows Lung Screening eligibility requirements.

  • Collects required data from patients for submission to ACR.

  • Assists with ongoing subject participation requirements.

  • Confirms shared decision making visit with PCP completed and order obtained.

  • Collaborates with patient to schedule LDCT (low dose CT scan).

  • Coordinates follow-up appointments, as needed, to ensure proper exams, tests, and pathology specimens are completed and submitted, as required by ACR.

  • Provides smoking cessation information and refers patients to the National Quit Line.

  • Collaborates with subjects, physicians, nursing staff, and other healthcare professionals to ensure overall satisfaction.

  • Provides written report of CT scan to patient and provider.

  • Schedules annual exams to ensure participant compliance.

  • Utilizes Lung Screening database to facilitate retrieval and communication of information.

  • Relays information to clinic staff and other departments as appropriate.

  • Maintains open communication with other departments in the health system.

  • Enters patient information into the database.

  • Performs skills competently and independently to assure patient safety.

  • Is accountable to learn new skills and equipment within the area's specifications and time frames.

  • Able to competently perform technical skills independently.

  • Works independently and is self-directed.

  • Demonstrates professional investment in departmental growth.

  • Joins GO2 Screening Centers of Excellence and reviews recommendations for continued improvement, maintaining our certification.
